We are looking to recruit a full time registered nurse to work within the Chesterfield Community nursing service, specifically in-reaching to the community support beds in the Staveley Centre, supporting patients to return to their own homes. Main duties of the job

The successful applicant/s will be responsible for providing evidence-based care to support patients in their own homes and in the Staveley Centre. The Staveley Centre is a unit of community support beds for patients discharged from hospital, aiming to return to their homes. To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital.

Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role..
