Position Summary

Responsibilities
Plan, implement, and evaluate nursing care.Assist RNs with specialized procedures.Provide care per plan of care and agency policy.Monitor and report changes in client condition.Document each visit thoroughly.Teach clients appropriate self-care techniques. Position Qualifications:
Current Colorado LPN or LVN License required.One year nursing experience in home health preferredValid Driver's License & clean Motor Vehicle Record (MVR) Benefits:
