Job summary
Lewisham is transforming neighbourhood care through the establishment of Integrated Neighbourhood Teams (INTs), designed to deliver coordinated, community-based care and support. The teams are made up of NHS services, primary care, social care, and the voluntary sector to provide more integrated and effective support for residents.
The four INTs align with Lewishams neighbourhoods and are closely integrated with six Primary Care Networks (PCNs). This approach builds on existing health and care service boundaries and proven models of successful collaboration in adult social care, children and young peoples services, district nursing, and mental health services.
Main duties of the job
The Health and Wellbeing Nutritional Coach will play a central role in supporting the implementation and delivery of Integrated Neighbourhood Teams by delivering the Lifestyle Medicine Service within Sevenfields and Ravensbourne PCN. This person will focus on improving outcomes for residents by providing holistic and personalised coaching and care which also helps to reduce local health inequalities.
Role holder must be an accredited nutritionist and hold an accredited coaching qualification.
Lifestyle medicine focuses on the prevention, treatment, and potential reversal of chronic disease through six key pillars: nutrition, physical activity, sleep, stress management, avoidance of risky substances, and social connection. This role involves providing one-to-one and group health and wellbeing coaching, with a particular focus on nutrition and lifestyle medicine, to support patients in making sustainable lifestyle changes. This service was co-designed with the local community to empower individuals with the knowledge and tools needed to proactively manage their health conditions. The service aims to enhance patient activation, improve health outcomes, and enhance quality of life through an evidence-based approach.

Sevenfields PCN and Ravensbourne PCN are located in the southern part of the borough, comprising six GP practices and delivering healthcare services to a population of approximately 71,000 patients. Together, the PCNs form Neighbourhood 3.

Population Health and Caseload Management
Manage and prioritise a caseload in accordance with the needs, priorities, and required support of individuals.Be responsible for managing patient referrals and waiting lists.Adapt service delivery to meet the diverse needs of the local population, ensuring inclusivity and accessibility.Work flexibly to meet the evolving needs of the service and patient population.Holistic Patient Assessments
Deliver one-to-one and group health and nutritional coaching sessions, incorporating all six pillars of lifestyle medicine.Provide personalised nutritional coaching to patients based on their unique goals and health challenges.Support patients in setting achievable health goals, improving self-management skills, and overcoming lifestyle-related health challenges.Use coaching techniques and structured frameworks to guide patients in problem-solving and behaviour change.Service Linkage and Signposting
Raise awareness of the Lifestyle Medicine Service, particularly among communities facing barriers to access.Work collaboratively within multidisciplinary and multi-agency teams to promote health coaching and serve as an ambassador for personalised care and supported self-management.Build and maintain strong relationships with GP practices, multidisciplinary teams, and community services to facilitate appropriate referrals and care continuity.
Integrated Working and Coordination
Act as the Lead Health and Wellbeing Coach for the Lifestyle Medicine Service, supporting the wider team where appropriate.Champion lifestyle medicine principles within the PCN and support integration across services.Participate in team, practice, and PCN meetings and contribute to the ongoing development of the service.Provide leadership on nutrition-focused interventions and guidance within the team.
IT and Communication
Collect and analyse patient feedback and service impact data to inform improvements and measure the effectiveness of coaching interventions.Actively contribute to data collection and reporting to evaluate service effectiveness.Engage in regular supervision and continuous professional development to enhance communication, coaching skills, and service delivery.
Community Engagement and Partnership
Engage with local communities to raise awareness of the service and promote inclusivity.Ensure the service is accessible and tailored to meet the specific needs of marginalised or underserved populations.Collaborate with community organisations to enhance patient support options and co-design service improvements.

The post holder is expected to work core hours but should also be flexible around service needs, which may include some evenings and weekends.
Person Specification
Qualifications Essential
Minimum of five GCSEs/O Levels or equivalent. Demonstrable commitment to ongoing professional development. PCI-accredited coaching qualification. Accredited nutritionist qualification.
Desirable
Willingness to undertake further training. Evidence of continued learning and development. Foundation Course or IBLM Diploma in Lifestyle Medicine.
Experience Essential
Experience working in a team environment. Experience in health and wellbeing coaching. Experience working as a nutritionist.
Desirable
Experience delivering lifestyle medicine approaches with patients. Experiencing of working with patients living with pre-diabetes and or diabetes. Experience in healthcare, social care, or community development settings. Experience working in multidisciplinary teams. Previous experience in the NHS or a similar healthcare setting. Familiarity with clinical systems such as EMIS. Experience with working with those with Learning Disabilities and SMI.
Skills and Abilities Essential
Excellent communication skills including written, verbal and use of IT. Motivational Interviewing and coaching skills. Ability to analyse and interpret information about health and well being.
