Role: Speech-Language Pathologist (SLP) — Home Health

Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) Minimum of 10 points per week

Location: Field-based

Comp: $82.50 / point

The mission of this roleDeliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

ObjectivesCarry a high-quality caseload: Minimum of 10 productivity points/week at target visit quality.

Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll ownOwn outcomes, not just visits: you are accountable for the patient’s SLP plan, progression, and functional outcomes.

Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits.

Assess and treat (as ordered/clinically indicated):

Aphasia and other communication disorders

Cognitive-communication impairment (attention, memory, executive function) impacting safety and independence

Dysphagia screening/assessment and swallow safety education (within home health scope and orders)

Build and execute a clear plan of care with measurable, functional goals.

Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

Coach patients/caregivers with practical strategies and routines that actually get done.

Document promptly and thoroughly — defensible, efficient, and consistent.

RequirementsActive Michigan ST license (in good standing)

Reliable transportation, valid driver's license, auto insurance

Home health experience strongly preferred

Able to carry/transport basic therapy equipment as needed

Competency in core home health PTA scope

Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

Strengthening, functional mobility, and home safety assessment/modification education

Patient/caregiver coaching with practical home programs
