At Prospect Hospice, we believe that everyone deserves outstanding, compassionate care at the end of life. Our highly skilled Single Point of Contact (SPoC) Team is the first point of contact for patients, families and healthcare professionals seeking specialist palliative care advice and support.
We are looking for an experienced and motivated SPoC Clinical Practitioner to join this innovative team providing expert clinical triage, holistic assessment and timely guidance to ensure that patients receive the right care, at the right time.

Main duties of the job
As a SPoC Clinical Practitioner, you will provide a central, telephone-based access point for hospice care ensuring patients and professionals receive timely, high-quality advice and support.
You will:
Triage referrals and assess urgency to ensure appropriate and responsive care Provide specialist clinical advice and emotional support via the advice line Undertake holistic telephone assessments to identify patient needs Support advance and anticipatory care planning Work collaboratively with multidisciplinary teams across health and social care Maintain accurate, confidential electronic patient records Contribute to clinical governance, audit and service development
This is a varied and rewarding role where your clinical expertise and decision-making will have a direct impact on patient care.
Hours: 22.5 hours per week, 9:00am - 5:00pm on a rota basis across Monday to Friday
Salary: £37,255 - £41,176 (FTE)

Prospect Hospice is an independent hospice supporting a community of 300,000 people in Swindon, Marlborough and North Wiltshire, and the villages of Lechlade and Fairford in Gloucestershire.
It is the only dedicated provider of specialist end of life care services within it's locality. The Hospices multi-professionals teams support people in the community, in care homes, at the Great Western Hospital and at the Hospice in Wroughton. Care is also extended to the families and friends of patients.

The Single Point of Contact Team is a highly skilled, innovative and adaptable team of health care professionals who are the first point of contact for all patients, family members and health care professionals who require specialist palliative advice, holistic patient needs assessment and support with advance care planning. We work with other health care professionals in primary and secondary care to provide oversight and guidance, to ensure best practice for patients and families, keeping the patient at the centre of care and allowing them autonomy in decision making.
What we are looking for;

Registered Nurses (NMC) Occupational Therapists, Paramedics or Allied Health Professionals (HCPC) You will have:
Relevant post-registration experience Experience or strong knowledge of palliative or end-of-life care Excellent communication and telephone triage skills Confidence in clinical decision-making and risk assessment A compassionate, patient-centred approachExperience in community, urgent care, or complex care environments would be highly beneficial.

Person Specification
Qualifications Essential
NMC Registered Nurse or Allied Healthcare Professional. Evidence of ongoing professional development within palliative care and willingness to undertake further study.
Desirable
Diploma/Degree in Palliative Care/Teaching qualification. Demonstrate the ability and willingness to complete relevant level 6/7 studies. This would include Patient Communication course, complexities of end-of-life care and remote clinical decision making.
Experience Essential
Relevany post registration experience. Experience in palliative care. Understanding of clinical governance.
Desirable
Experience in community care. To have an understanding and adequate knowledge base regarding effective symptom control management within palliative care and confidence to make safe recommendations for symptom control.
Personal Qualities Essential
Commitment to and ability to demonstrate Prospect Hospice values and behaviours at all times. Have a track record of working inclusively and a genuine appreciation of the value of diversity. Skills Essential

Advanced telephone communication skills-managing sensitive information, responding to caller distress. Management and organisational skills.
