Job summary
The Advanced Nurse Practitioner is responsible for the delivery of the nursing and medical needs of patients with both acute/chronic disease through proactive case management of patients conditions including liaison with both internal and external professionals and other agencies to manage the patient effectively. The post will also include telephone triage and initial contact for patients requiring urgent (on the day treatment) The Advanced Nurse Practitioner is directly responsible to the GP Partners but works in partnership with all clinical staff as part of a cohesive and efficient clinical team.

Our multidisciplinary team consists of; 5 GP Partners, 2x Salaried GPs, 1 Managing Partner, 2x ANPs, 1x Clinical Pharmacist, 1x Pharmacy Technician, 3x Practice Nurses, 2x HCAs. The team is supported by a hardworking administration team. We are an accredited training practice GP training, FY doctors and medical students on placement.
We are paperless utilising EMIS Web, Docman, Accurx, PATCHs and Electronic Prescribing and are high achievers in both national and local performance frameworks and enhanced services. We have a fully digitised Lloyd George records and operate a workflow redirection system to minimise administrative burden.

Job description Job responsibilities
Main Duties and Responsibilities
1. To write, and maintain policies within the practice for clinical areas and to oversee the development of policies written by the practice nurses
2. To promote best practice in the implementation of such policies
3. Identify, monitor and review deficiencies in procedures and implement remedial action
4. To work with colleagues to ensure contractual targets are met
5. Provide a first point of contact in the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making to establish a diagnosis and management plan
6. Instigate necessary diagnostic tests or investigations and interpret findings/reports
7. Prescribe any medicine for any condition within their competence (including some controlled medicines) provided they have completed an Independent Prescribing qualification
8. Undertake appropriate home visits
9. Instigate radiology requests ensuring training in accordance with the local hospital trust
10. Monitor indicators of chronic disease anticipating possible decline and proactively manage to enhance well-being and maintain independence
11. Develop evidence based specialised care plans for patients
12. Plan interventions in light of patients expressed wishes and goals of care
13. See patients with undiagnosed, undifferentiated medical conditions and make treatment decisions
14. Evaluate possible treatment options as diseases progress
15. Refer patients directly to other services/agencies as appropriate
16. Admit patients directly to secondary care hospitals in acute medical need
17. Communicating effectively with patients and carers, and recognising their needs for alternative methods of communication
18. Provide expert advice to patients
19. Undertake consultations to a high standard in the surgery, over the telephone and in the patients own home
20. Undertake consultations to a high standard in the surgery via video consultations.
21. Take a lead on care home patients offer digital ward rounds or face-to-face reviews as required and authorised by the partners
22. Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
23. Know the general principles of first aid and resuscitation to be able to undertake initial actions as appropriate
24. Provide teaching to e.g. existing staff, HCAs, practice nurses, nurse practitioners, medical students, GP registrars
25. Provide leadership and advice to the nurse practitioner, practice nurses and healthcare assistant supporting them to develop and maintain their clinical skills
26. Carry out audits and research as required and monitor own practice
27. Attend and participate in any Practice meetings
28. Develop the nursing service provided by the practice
29. Help develop and set up new patient services and participate in initiatives to improve existing patient services
30. Develop own professional knowledge and skills
31. Maintain professional registration
32. Work in accordance with the latest NMC code of professional conduct
33. To adhere to all practice policies procedures and guidelines
34. To comply with the standards set out in the contract of employment
35. To undertake any other duties as appropriate to the role
Person Specification
Qualifications Essential
Registered General Nurse (Currently registered with the Nursing & Midwifery Council) Masters Degree or equivalent to meet RCN recommendations for advanced practice Independent Nurse Prescriber qualification
