Job summary
We are seeking an enthusiastic and highly motivated Senior NMC Registered Community Staff Nurse to join our friendly and innovative Community Nursing Team covering South and East ambulatory clinics.
We have a great interest in moving Community Nursing forward to support our local population and service needs and we are looking for a highly skilled Community Nurse to lead the team to deliver this effectively. You will take responsibility for the planning and delivery of care and the management of staff in the absence of the Clinical Lead/Operational Lead. You must be able to demonstrate managerial and leadership skills and be able to make sound clinical decisions independently.You will work alongside the leadership team to ensure proactive and effective care delivery, liaising and working with the multidisciplinary team to ensure individualised and evidence-based care to our patients.
You will work alongside our Lead Specialist Practitioners, Operational Leads and District Nurses to provide effective leadership, support staff development and caseload management.
You will be required to travel independently over a large geographical area.
Main duties of the job
As Senior Nurse, you will manage a caseload, alongside the Lead Specialist Practitioner to provide support and supervision to staff to enable provision of high-quality person-centred care to patients within their own homes.
As part of this role, you will be expected to:
Hold a key role in building and maintaining excellent working practices with Social Care and Allied Health colleagues within the primary care network. Be an effective leader, demonstrating care, compassion and empathy and role modelling the Trust values. Be competent in a variety of clinical skills, and be able to share your knowledge and expertise with others, signposting to more specialist services as appropriate. Engage complex patients, agree mutually beneficial treatment plans and to plan, implement and evaluate care pathways. Support with quality assurance, including audit, incident reporting and gaining patient feedback. You will work with the leadership team to evaluate this in line with objectives. Looking for someone with excellent clinical, communication, interpersonal and team skills to work collaboratively across all care settings.
