Job summary
We are looking for a motivated and team-focused Advanced Paramedic Practitioner to join our friendly forward-thinking practice caring for approximately 18000 patients.
Main duties of the job
This is a full-time role which will incorporate minor illness clinics, clinical triage and assessment plus some home visits and care of housebound patients.
The role will be varied and will develop around the individual as well as practice needs.

Key Responsibilities
Clinical Assessment & Care: Perform home visits, assess undifferentiated patients and treat minor illnesses /injuries.
Patient Management: Diagnose acute and chronic conditions, prescribe medications and manage patient care plans
Urgent Care Response: Provide rapid, expert care to patients, including telephone triage and home based interventions to prevent hospital admission.
Primary Care Support: Work within GP practices or Primary Care Networks (PCNs) to manage complex caseloads.
Health Promotion: Advise patients on health maintenance self-care and medication management.
Person Specification
Qualifications Essential
Degree in Paramedic Science or Equivalent MSc Level advanced clinical practice education
Desirable
Non-Medical prescribing qualification
Experience Essential
Minimum of 3 years post graduate experience Advanced assessment skills to include 12 lead ECG interpretation, phlebotomy, swab taking and other investigative tests interpretation
Desirable
Experience of working in primary or secondary care
