Job summary
The Patient Safety Incident Response Strategic Lead is a varied and high-profile role. The post holder will lead the Trust's response to the most serious and complex patient safety events, providing expert investigation and review, and supporting patients, families and colleagues throughout the process.
The post holder will coordinate thematic reviews and ensure learning from patient safety events is translated into sustainable improvement across the organisation. The post holder will also provide oversight and quality assurance of investigation documentation and reports, and will provide support to more complex After Action Reviews (AARs) which are led by the Clinical Directorates.
The post holder will also undertake the PSIRF Engagement Lead role for relevant responses, ensuring compassionate involvement of patients, families and carers, supporting delivery of Duty of Candour, and promoting a psychologically safe learning environment underpinned by just culture principles.
Please note, this post is for 22.5 hours per week.
Main duties of the job
Lead on our PSII investigations and thematic reviews Provide expert advice and guidance to staff leading other learning responses Provide training to staff within the organisation of PSIRF principles / systems thinking to assist with the embedding of other learning responses Analyse incident and investigation themes to identify trends and learning opportunities, contributing to Trust-level reporting (monthly/quarterly/annual) and informing prioritisation of improvement work. Be one of the formal named lead Patient Safety Specialists in the organisation leading on or directly supporting patient safety improvement activity, ensuring that systems thinking, human factors understanding and just culture principles are embedded in all patient safety processes across HDFT.
