Main duties of the job

The Pharmacy Technician will play a key role in supporting medicines optimisation and ensuring safe, efficient prescribing processes within the practice.

Day-to-day work will include managing prescription requests, reconciling medications following hospital discharge, and responding to medication-related queries from both patients and the wider clinical team. The role involves maintaining accurate clinical records and supporting safe prescribing through attention to detail and good governance.

The postholder will contribute to service improvement by supporting audits, monitoring prescribing activity, and assisting with quality initiatives linked to QOF, PQRRS and medicines optimisation programmes.

Working closely with GPs, pharmacists and the wider multidisciplinary team, this is a varied and evolving role offering the opportunity to develop skills, take ownership of key processes, and contribute to high-quality patient care.

Job description Job responsibilities

The Pharmacy Technician will support the delivery of safe, effective and patient-centred medicines optimisation across the practice, working as part of a multidisciplinary team.

The role includes supporting patients to use their medicines effectively, assisting with medication reviews and medicines reconciliation following hospital discharge or care transitions, and helping to coordinate monitoring requirements for structured medication reviews. The postholder will contribute to improving prescribing safety, reducing medication waste, and supporting antimicrobial stewardship.

You will work closely with GPs, pharmacists, care homes and community pharmacies to ensure continuity of care, including managing shared care processes and supporting patients with long-term conditions.

The role also involves improving prescribing systems and processes, promoting efficient ordering and use of the Electronic Prescription Service (EPS), and supporting colleagues with medicines-related queries. There is a strong focus on collaboration, service improvement, and contributing to wider health outcomes for the practice population.

computer literate knowledge of emis or other clinical systems primary care experience / general practice experience and training
