As a Care Coordinator, you will play a key role in supporting the smooth day-to-day running of patient care within the practice. You will be responsible for organising and tracking patient journeys, ensuring appointments, referrals, and follow-ups are managed in a timely and efficient manner. The Care Coordinator will act as a central point of contact for patients, assisting them in navigating healthcare services and ensuring clear and timely communication between patients, clinicians, and external providers. The post holder will play a key role in supporting the organisation and management of patient care plans, appointments, referrals, and follow-ups, helping to ensure continuity and efficiency in care delivery.

Main Responsibilities
