Job summary
We are looking to recruit an enthusiastic, motivated and patient-focused Pharmacist to join our friendly and supportive primary care team.
We currently care for approximately 10,000 patients, with the management team also responsible for two further practices, Mayflower Medical Practice and Field Road Surgery, serving approximately 7,000 and 10,000 patients respectively.
This is an exciting opportunity for a pharmacist to become an integral part of our multidisciplinary primary care team, working alongside GPs, practice staff and other healthcare professionals to provide high-quality, safe and effective medicines management.

Main duties of the job
The successful candidate will work as an autonomous practitioner within the primary care multidisciplinary team, providing high-quality clinical pharmacy services to the practice population.
Key responsibilities will include:
Providing expert clinical advice on medicines optimisation and prescribing
Conducting structured medication reviews and supporting patients with their medicines
Identifying and resolving medication-related problems
Supporting safe, effective and cost-effective prescribing
Reviewing repeat prescriptions and supporting appropriate medicines management
Providing advice to clinicians and other healthcare professionals regarding medication and prescribing queries
Supporting the management of long-term conditions through medicines optimisation
Supporting the practice with medication monitoring and appropriate follow-up
Contributing to prescribing audits, quality improvement and service development
Working collaboratively with GPs, nurses, reception/admin teams and other members of the multidisciplinary team
Maintaining accurate clinical records and working within professional and regulatory standards
The post holder will be expected to work collaboratively within the practice team, demonstrating a patient-centred approach and contributing positively to the ongoing development of pharmacy services within primary care.

The practice has a Good rating High QOF achievements Based in a purpose built building with generously sized consulting rooms. Uses Systm1 The practice has a full-time pharmacist supported by a network pharmacist who is responsible for medicines management and workflow which reduces GP workload. We are member of a proactive primary care network and Primary Care Doncaster. Host both student nurses and medical students with aspirations to become a postgraduate GP training practice.

The post holder will work within their professional scope of practice and adhere to the standards and guidance set by the General Pharmaceutical Council (GPhC). They will contribute to the effective management and development of pharmacy services within the practice, working collaboratively with GPs, nurses, other healthcare professionals and the wider multidisciplinary team.
The successful candidate will be expected to demonstrate a commitment to patient safety, clinical excellence and continuous improvement, while contributing positively to the practice and supporting the delivery of high-quality primary care services.

Qualifications Essential
Registered Pharmacist with the General Pharmaceutical Council (GPhC) Appropriate pharmacy qualification (MPharm or equivalent) Evidence of ongoing Continuing Professional Development (CPD) Experience of working within a healthcare or primary care environment Understanding of medicines optimisation and safe prescribing Understanding of clinical governance, patient safety and risk management Knowledge of relevant pharmacy legislation, professional standards and guidance Understanding of the role of pharmacy within primary care and the wider multidisciplinary team

Postgraduate qualification in clinical pharmacy, primary care pharmacy or equivalent Experience of working within a GP practice or Primary Care Network Experience of medicines management and optimisation Experience of supporting long-term condition management Knowledge of QOF, NICE guidance and local prescribing policies Experience of ICB initiatives and primary care pharmacy programmes Experience of clinical audit and quality improvement
