About this role

Job summary

Cumberland House Surgery is currently looking to recruit an Advanced Nurse Practitioner to add to our existing clinical team. The post is intended to be 32hrs per week worked over 4 days but flexible for suitable candidates.

We are an established Practice looking after approximately 9500 patients over a wide geographical area. We are a forward thinking and innovative GMS Practice, situated in the thriving market town of Stone.

Main duties of the job

You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat or refer patients.

Carry out consultations with patients taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis.

Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate.

Provide clinical advice and mentorship to junior clinical staff.

Travel to patient homes and nursing homes and subsequent management of clinical problems, both acute and long-term.

Long term conditions management in the Practice setting, in accordance with established protocols based on current clinical guidelines.

Liaison with patient relatives where appropriate.

Be able to work within the latest NMC Code of Professional Conduct and to be conversant with the scope of professional practice and other NMC advisory papers.

Advanced Nurse Practitioner

Job Summary

Cumberland House Practice is currently looking to recruit an Advanced Nurse Practitioner, to add to our existing clinical team. The post is intended to be 32 hours per week and permanent but we are flexible and happy to consider part-time hours. We are an established practice in the canal town of Stone, Staffordshire looking after approximately 9500 patients over a wide geographical area.

Main Duties and Responsibilities

Working as part of the healthcare team, which includes the GPs, Practice Nurses and Healthcare Assistant and other healthcare professionals that are integrated into the team

Management of acute medical conditions, within the scope of the role holders experience and competence, with support from doctors when required

Liaison with the GP on-call whenever appropriate

Travel to patients homes and nursing homes and subsequent management of clinical problems, both acute and long-term

Long-term conditions management in the Practice setting, in accordance with established protocols based on current clinical guidelines

Liaison with patients relatives where appropriate

Refer patients to other services and healthcare providers when clinically required.

The candidate should

Be able to act pro-actively to improve patient care Have good communication skills with patients, relatives and colleagues Be able to work independently, but be aware of own limitations and competence levels in order to seek the support from GP colleagues that will be available Be motivated to improve own skills and follow principles of Personal Professional Development Be able to deliver a high standard of patient care as an Advanced Nurse Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Have a good understanding and appreciation of clinical governance and to work within ones competence. Instigate necessary diagnostic tests or investigations and interpret findings and reports Be able to prescribe safe, effective and appropriate medication as defined by current legislative framework and considering agreed prescribing formula guidelines. Be able to maintain comprehensive, accurate and contemporaneous computerised records of all consultations and contacts with patients and, when appropriate, written or printed ones. PERSON SPECIFICATION:

Advanced Nurse Practitioner: General Practice

ESSENTIAL

DESIRABLE

Qualifications:

Registered General Nurse (Currently registered NMC)

Recognised NP qualification at minimum of degree level or equivalent

Independent Nurse Prescriber

Teaching / Mentoring experience and /or qualification

MSc in nurse practitioner studies or equivalent

Experience:

Minimum of 5 years post registration experience

Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the NP role

Experience in management of long term conditions e.g. asthma, COPD, diabetes, CHD.

Evidence of working autonomously and as part of a team

Proven ability to evaluate the safety and effectiveness of their own clinical practice

Previous experience of managing and developing a nursing team

Interpreting and implementing local and national policy agendas for health

Knowledge :

Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS contract, Clinical Governance, Quality & Outcomes Framework

Knowledge of national standards that inform practice (eg.,National Service Frameworks, NICE guidelines etc)

Understanding of their accountability arising from the NMC Code of Professional Conduct (2004) and medico-legal aspects of the Nurse Practitioner role

Understanding of equal opportunity and diversity issues

Skills:

Ability to assess and manage patient risk effectively and safely

Excellent interpersonal, verbal and written communication skills

Reflective practitioner

Time management and ability to prioritise workload

Able to analyse data and information, drawing out implications for the individual patient/impact on care plan
