Job summary
The Virtual Health consisting of Telemedicine, Clinical Communication Centre and Virtual ward are integral parts of the trust clinical flow to ensure all patients receive the right care at the right place at the right time. they are parts of the trust left shift to ensure patients are cared for in the community where possible
Main duties of the job
The Virtual Health Hub has just received substantive funding after a successful pilot to the Telemedicine, Clinical Communication Centre and Virtual ward.
What we are looking for:
Experienced clinicians with clinical knowledge in the care and management of the acutely ill/complex care/palliative care/intermediate care patient within and outside the hospital.
Excellent communication skills.
Highly self-motivated.

Person Specification
Training and Qualifications Essential
Professional UK registration e.g. NMC, HPC, GPhC Degree level education or equivalent knowledge and skills gained through any combination of alternative study, or employment experience Able to demonstrate a good command of the English Language both written and verbal
Desirable
Additional Post graduate learning in specialist field including master's degree or working towards Project management learning and/or development Communication skills learning and/or development
Experience and Knowledge Essential
Evidence of Continued Professional Development (CPD) clearly recorded for professional profile Extensive clinical knowledge in the care and management of the acutely ill/complex care/palliative care/intermediate care patient within and outside the hospital Evidence of post registration clinical work experience in the speciality
Desirable
Experience of change management Experience of project management for policy development or large scale projects Experience of remote video assessment or telephone triage service
Skills and Ability Essential
To demonstrate the required behaviour in keeping with the Trust values Organisational and on-going planning skills including own workload and of others in an unpredictable environment Understanding of effective clinical governance including implications, quality and audit
Desirable
Proven remote video assessment skills The ability to work at problems from a unique or different angle.
