About this role

Job summary

The Experienced General Practice Nurse plays a pivotal role in delivering safe, high-quality, and person-centred care. This role is integral to achieving the objectives of the 2026/27 Network Contract DES, with a focus on continuity of care, health inequalities reduction, digital transformation, and long-term condition (LTC) optimisation.

Full-time (37.5 hours per week), or part-time (minimum of 20 hours per week) considered in line with service requirements.

This role is included within the extended hours provision and will involve working an evening on a rota basis.

Main duties of the job

The postholder will work as part of the existing practice nurse team on long-term condition management, champion public health and prevention, and embed population health principles within general practice.

They will support the development and supervision of new-to-practice nurses and other multidisciplinary staff, contributing to workforce capability across the Primary Care Network. The role also involves fostering integration with community, secondary, and social care systems to ensure proactive and joined-up care delivery.

The post holder will be working at registered nurse level practice as detailed in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.

About us

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.

One of 13 PCNs in Somerset, we are a collaboration of five GP practices in West Mendip: Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice, Wells Health Centre and Vine Surgery Partnership (Street).

At Wells City Practice, you'll be joining a friendly, supportive team committed to providing high-quality patient care. Serving around 8,500 patients, the practice has a 'Good' CQC rating, high levels of patient satisfaction and strong QOF achievement.

Our multidisciplinary team includes six partners, two salaried GPs, a paramedic practitioner, nurses, HCAs, FCP, Clinical Pharmacist and an experienced administrative and management team. We are actively involved in training, research and collaborative working through our local Primary Care Network. Job description Job responsibilities

KEY TASKS AND RESPONSIBILITIES

Work within the multidisciplinary team on long-term condition management across the PCN, ensuring evidence-based, standardised care aligned with QOF, IIF, and local priorities Support practice teams to identify at-risk patients and implement proactive care plans Support public health and screening programmes, including delivery and uptake of immunisations, vaccinations, and cervical screening. Work with the PCN team to achieve national targets and reduce variation in outcomes Mentor, support, and supervise new-to-general-practice nurses, student nurses, and other healthcare professionals e.g., nursing associates, HCSWs across the PCN, contributing to induction, professional development, and appraisal processes Work collaboratively across the PCN, actively engaging with community services, secondary care, social care, and voluntary sector providers to ensure integrated and holistic care pathways Undertake mandatory training and CPD to meet NMC revalidation and practice needs Reflect on learning using appropriate frameworks e.g. NMC Code Adhere to CQC standards, infection prevention and control protocols, and safeguarding responsibilities Participate in audits, significant event reviews, and quality improvement initiatives Maintain confidentiality, information governance, and data protection in line with NHS standards Recognise and work within own competence and professional code of conduct as regulated by the NMC Ensure that their own practice is kept updated, using an acceptable model of clinical supervision Take every opportunity to expand individual practice in line with the principles contained within the Nursing and Midwifery Councils document The Code 2015 or equivalent professional body Communicate with enthusiasm and conviction; in a style that, when relevant, motivates, inspires, and encourages Create a trusting partnership with patients and/or relatives/carers to communicate and explain complex medical issues - including new diagnoses and agree a management/treatment plan Provide nursing treatments to patients in participation with doctors or independently to agreed protocols. This would include: o Dressings, care and treatment of minor skin disorders and wounds including removal of sutures.

o Routine intramuscular and subcutaneous injections

o Ear syringing

o Taking blood, scrapings and swabs for laboratory investigation

o Taking cervical smears

o Perform ECGs

o Monitor Warfarin levels via INR Star

o Assist doctors with minor operations

o Take part in immunisation campaigns e.g. flu

o Provide health screening, advising on general healthcare and treatment of minor ailments, referring to a doctor where appropriate

o Keep and maintain patient records via the computer

o Undertake and attend regular appropriate training courses, updates and meetings

Experience Essential

Demonstrable experience in long-term conditions management and quality improvement Supports public health and screening programmes, including immunisations, vaccinations, and cervical screening Supports, mentors, and supervises new-to-practice nurses, students and other healthcare professionals Experience in identifying and supporting vulnerable patients, including mental health and safeguarding cases

Qualifications Essential

Registered with the NMC and maintains revalidation in line with NMC requirements Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework

Additional qualifications in long-term condition management Experience and understanding of health inequalities for the local population Holds more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives Relevant qualification, accredited training, or a keen interest in diabetes management Mentor qualification (ENB 998 or equivalent)
