About this role

This role offers the opportunity to work within the multidisciplinary diabetes team providing support for people with Diabetes across Somerset. The post is for 30 hours working between Monday to Friday, days will be agreed with the successful candidate.

Person Specification

Qualifications Essential

Qualifications and Credit Framework (QCF) Level 3. This could be a Vocational NVQ, BTEC or equivalent international qualification in healthcare or able to demonstrate the equivalent level of experience Ability to prioritise work, to work independently and as part of a team.

Desirable

Understanding of Diabetes as a Chronic condition and potential for associated health complications. Knowledge of clinical audit processes

Experience Essential

Experience of working in a healthcare setting Experience of lone working, working on own initiative. Experience of working in a team.

Desirable

Experience in patient facing roles and caring for people with additional needs. Awareness of NHS supply ordering and procurement process

Additional Criteria Essential

Willingness to use technology to improve standards of care and support to our patients Ability to work flexibly to meet the demands and requirements of the role. Hours usually Monday- Friday, very occasional evening work to facilitate Excellent Organisational skills

Desirable

Basic counselling skills training. Experience in Excel spreadsheets
