Job summary

Barnstaple Alliance Primary Care Network (PCN), on behalf of One Northern Devon, is recruiting a Health Inequalities Project Lead to support the delivery of projects aimed at reducing health inequalities and bringing about positive change across North Devon.

The successful candidate will work as part of the wider One Northern Devon Partnerships team, leading and supporting a range of projects and activities to ensure agreed objectives and outcomes are achieved.

Hours: 18-25 hours spread over the week to meet the needs of the role, to be agreed with the successful applicant

Salary: £16.00-£19.20 per hour depending on experience and qualifications

Contract: 12-month fixed-term project

Location: Based in Litchdon and Brannam Medical Centre and some home working

Main duties of the job

The Health Inequalities Project Lead will lead and contribute to the effective delivery of health inequalities projects across North Devon, acting as an enabler and champion of change.

The role will involve developing and delivering project plans, analysing and presenting data, coordinating meetings, workshops and events, engaging with stakeholders and voluntary, community and social enterprise organisations, writing project reports and supporting grant funding applications.The post holder will work in line with the One Northern Devon reducing health inequalities strategy, with a particular focus on workstreams including Closing the Gap, Fuel Poverty, Humanising Services and the equity-focused model of Neighbourhood Health.The role will require an understanding of the wider factors contributing to health inequalities and the ability to use project management, data and partnership working to support improvements in health outcomes across North Devon.

Barnstaple Alliance Primary Care Network (PCN) is a proactive and supportive network of four General Practitioner surgeries: Brannam, Fremington, Litchdon and Queens Medical Centres. Together, we provide care to over 52,000 patients across the Barnstaple area. Whilst retaining our individual practice identities, we work collaboratively to improve and develop healthcare services for our local population.

This role is hosted by Barnstaple Alliance PCN on behalf of One Northern Devon and will work as part of the wider Partnerships team. One Northern Devon brings together organisations and partners across North Devon to tackle health inequalities and improve outcomes for local communities. The Health Inequalities Project Lead will work on behalf of One Northern Devon as part of the wider Partnerships team, supporting the delivery of the One Northern Devon reducing health inequalities strategy.

The role will focus on the following:

Leading on developing project plans and objectives in line with scope and budget to meet the needs of the organisations and programme. Employing a variety of project related tools, techniques and best practice methodologies. Employing business and data analysis techniques to support project objectives. Interpreting data and highlight variation to support open discussion about where variation is and is not warranted. Effectively present and communicate data. Setting up, coordinate and facilitate meetings, workshops and events. Taking minutes, creating action plans and follow up on actions. Documenting and tracking the benefits to be realised during and after project implementation, ensuring project delivery is focused on benefits realisation. Writing project reports, including updates and outcomes of completed project. Completing and managing the application processes for grant funding bids. Maximising stakeholder engagement opportunities in the course of project activities. Building and maintaining relationships with VCSE. Reporting of risks and issues in line with the Partnership risk and issue process. Ensuring and promoting use of programme governance structures and processes to support project activity and provide a high level of assurance to the organisations.

Having a good understanding of the broader drivers that contribute to health inequalities within the North Devon population and use this knowledge, along with the use of data management, to support the improvement of health outcomes. Communicating Health Inequalities strategy including Closing the Gap and Core20PLUS5 to stakeholders. Having an understanding of wider policy frameworks such as Neighbourhood Health and impacts on health inequalities.

Experience Essential

Experience of project/change management Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers available to support the Digital and Transformation Lead role Experience of producing agendas and minutes for meetings Report writing

Qualifications Essential

Good standard of education with excellent literacy and numeracy skills Degree/Leadership and/or Management Qualification or proven leadership experience Project management qualification, such as PRINCE or project management equivalent experience
