About this role

Job summary

We are seeking a skilled, motivated and compassionate Community Practitioner to join the Newcastle & Gateshead Step Up Hub. This is an exciting opportunity to work within a dynamic multidisciplinary team, supporting individuals to remain safely within their community and avoid unnecessary hospital admission. Applicants must be registered nurses with a current professional registration.

This post offers a rewarding mix of community and hospital in-reach responsibilities, with a focus on delivering high-quality, rehabilitation-oriented care.

In this role, you will provide care coordination and nursing support to service users with complex mental health needs across both community and inpatient settings. You'll play a crucial part in ensuring smooth transitions between hospital and community care, attending Mental Health Act tribunals and working closely with multidisciplinary teams.

You will need to be clinicallyexperienced, solution focussed and competent in working with individuals, their families, and carers. You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

Main duties of the job

Deliver high-quality, person-centred mental health care in both community services Undertake comprehensive risk assessments and care planning Attend Mental Health Act tribunals and professionals' meetings as required Prepare formal clinical reports for tribunals Work collaboratively with inpatient teams to facilitate discharge planning and continuity of care Work collaboratively with your service users and relevant individuals/professionals they have involved in their care. Provide support and input during mental health crises and implement robust risk management plans Mentor and support junior staff, students, and colleagues Liaise effectively with GPs, social workers, carers, and other services

Person Specification

Education and Qualifications Essential

Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist Mentor Training Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post. Proven track record of post registration clinical experience relevant to the post. Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection. Up-to-date knowledge/experience of relevant legislation.

Desirable

Knowledge of evidence based treatments for psychosis and non psychosis Experience of working within a community team.

Knowledge and Experience Essential

Awareness and understanding of professional accountability, NMC standards and other local and national initiatives. 12 to 24 month band 5 qualified nurse experience dependent upon service need.

Experience of working within a community mental health setting

Skills and Competencies Essential

Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs. Risk Assessment and Risk Management / Safety Planning skills. Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance). Demonstrate effective written and verbal communication skills. Evidence of a high level of professional, organisational and leadership skills (self & others). Ability to adapt within a changing environment.
