Job summary
An exciting opportunity has arisen within the Urgent Community Response Directorate to recruit two Community Case Managers within the Virtual Ward Team.
We are seeking to appoint a dynamic, driven, motivated and caring individual who will bring their experience and personality to the role to ensure that as a team we align to the Trusts People, Patient, Population Strategy.
The role would suit someone with recent Emergency and Urgent Care experience within a Community setting.
Our current Virtual Ward Team consists of a range of multidisciplinary colleagues including:
Consultants
ACPs
Occupational Therapists
Physiotherapists
Community Matrons
Assistant Practitioners
Rehab Support Workers
Operational Managers
We currently have 6 Virtual Wards within the organisation including our award-winning Hospital at Home Service (EPCENTRE) lead in collaboration with our Acute Medicine colleagues. Working with the team, you will support across 4 of the Virtual Wards as detailed below:

Main duties of the job
Reporting to a senior Community Matron for the service, the post holder will be an autonomous practitioner and clinical specialist responsible for developing and implementing service provision within the team across the borough of Sandwell.
The post holder will have continuing responsibility for proactively monitoring and managing patients on a caseload within a designated pathway, recognising the early symptoms of disease exacerbation, acute illness, and injuries, as well as social and familial factors based on the understanding of long-term conditions, the effects on the individual, their family and environment as well as current evidence and practice standards.
The service will be delivered to a defined population of patients needing complex case management for long term conditions and complex social risk factors within the community setting, preventing inappropriate hospital admission.
The post holder will also be ultimately responsible for the management/decision making about care and service provision, for individual patient pathways across the primary and secondary care interface.

Registered General / Adult Nurse. Qualified Nurse Prescriber or willing to study for. Hold a recognised teaching/assessing qualification. Evidence of continued professional development. Relevant qualification/ module at first degree level.

Post registration experience working in community or acute care group. Leadership Essential
Able to speak, receive and issue instructions concerning patients without risk of misunderstandings in group and individual settings. Able to communicate effectively in face to face contact, telephone contact, e-mail contact and internal and external meetings. Able to carry out frequent, long periods of medium-intense concentration carrying out assessments, planning treatment programmes, supervising support workers and more junior staff and dealing with complex equipment Able to deal sensitively with patients with high levels of anxiety/aggression which may be due to dementia, sedation, limited mobility or who are unable to accept current clinical outcomes.
