About this role

Job summary

A Practice Nurse plays a key role within a General Practice, delivering highquality, patientcentred nursing care as part of the primary care team. They work closely with GPs, Nurse Practitioners, Health Care Assistants, and wider multidisciplinary colleagues to support the health and wellbeing of the practice population

Main duties of the job

Main ResponsibilitiesClinical Duties

Chronic disease management:Diabetes

Asthma & COPD

Hypertension & cardiovascular disease

Chronic wound care

Treatment room duties:Wound management and dressings

Ear irrigation

ECGs

Urinalysis

Cervical screening (if trained)

Immunisations:Childhood vaccinations

Travel vaccines

Seasonal flu & COVID19 vaccinations

Health checks and assessments:NHS Health Checks

Wellwoman and wellman clinics

Smoking cessation advice

Weight management and lifestyle interventions

Quality & Safety

Maintain accurate clinical records in line with practice policies

Follow infection control standards

Participate in audits, QOF, and clinical governance activities

Support safeguarding processes for adults and children

1. Chronic Disease Management

Provide structured reviews for long term conditions such as:

AsthmaCOPDDiabetesHypertensionCardiovascular diseaseDevelop personalised care plans and provide patient education.2. Treatment Room DutiesWound assessment and managementDressings and suture removalEar irrigationECG recordingUrinalysisBlood pressure monitoringContraceptive advice (nonprescribing)3. Immunisation & VaccinationChildhood immunisationsTravel vaccinationsSeasonal flu and COVID19 vaccinationsAdult vaccinations as per national schedules4. Health Screening & Preventative CareCervical cytology (if trained)NHS Health ChecksLifestyle advice: smoking cessation, weight management, alcohol advicEHealth promotion within the practice population5. Clinical Quality & GovernanceMaintain accurate, timely clinical recordsContribute to QOF, audits, and quality improvement projectsAdhere to infection prevention and control standardsSupport safeguarding processes for adults and childrenWork in line with practice policies, guidelines, and professional standards6. Person Specification

Qualifications Essential

Person Specification Essential Registered Nurse (NMC) Experience in primary care, community, or acute nursing Immunisation and vaccination training Chronic disease management knowledge Ability to work autonomously Strong communication and interpersonal skills IT literacyClinical System essential

Desirable

Cervical cytology training Chronic disease qualifications (e.g., diabetes, respiratory) Experience using EMIS Mentor/teaching experience Interest in quality improvement or audits

Experience Essential

Experience of working within primary care and a nursing team

Desirable

Dealing with vulnerable patients Experience of working in a GP practice
