key responsibilities
provide clinical leadership across supported living services review and implement care plans for individuals with complex or high risk needs act as the lead clinical liaison for referrals transitions and mdt meetings support service managers to embed best practice and maintain cqc compliance monitor incident reports safeguarding alerts and health outcomes lead on health audits clinical risk assessments and medication reviews train and mentor support staff in traumainformed highquality care work collaboratively with local authorities nhs commissioners and families
Person Specification
Experience Essential
minimum 3 years post registration experience in a clinical or Care Coordination role experience in supported living residential care or community adult health
Desirable
Strong understanding of CQC, DoLS, MCA, and risk management frameworks. Excellent communication, leadership, and organisational skills. Ability to travel between services (mileage reimbursed).
Experience Desirable
Experience with forensic clients or dual diagnosis Experience supporting services through cqc inspections
Qualifications Essential
NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
