We are looking for a Home Treatment Team Practitioners to be part of our motivated and experienced teams.While working with the team you will gain experience in relation to risk assessment, risk management, care planning, engagement andcommunication withClients who are experiencing a mental health crisis and in need of support in their own homes.You will be expected to provide a high level of care and have excellent communication skills.In return our service will provide you with supervision, training and support to enable you to fulfil the role and provide you with a personaldevelopment plan to help you advance in your career.The Operational Directorate is committed to the principle of learning from those who use our services, carers and families. Main duties of the job
Main duties of the jobWorking autonomously within the parameters of the role leading and supportingjunior staff and wider care team to implement the delivery of quality nursingcare, contributing to the physical, psychological, social, recreational andspiritual care in a recovery-focused way through therapeutic engagement andactivities, and taking steps to address any issues that arise.. Leading on and undertaking activities to ensure that the fundamentals of careare met - nutrition, hydration, hygiene, comfort, emotional and social support:o Co-ordinating and evaluating collaborative patient centred care; gettingfeedback and improving plans with the multi-disciplinary team (MDT).o Providing accurate information about care in an accessible format to patientsand their relatives / carers.o Liaising and working positively with members of the multi- disciplinary teamand partner organisations.o Administering, and monitoring the side-effects of, medication; monitoringservice user compliance.o Safety; safe custody of medicines, sharps, clinical equipment, report allincidents following trust policy.Leading and supporting/teaching junior staff on risk assessment and riskmanagement that involves making decisions based on knowledge of theresearch evidence, knowledge of the individual service user and their socialcontext, knowledge of the service user's own experience and clinical judgment.

The Older Adults Home Treatment Teams (HTT's) provide comprehensive and accessible crisis resolution and home-based care and treatment topeoplein the acute phase of mental illness in their own homes as an alternative to hospital admission.The primary function of the teams is to provide intensive treatment in the persons own home or other suitable environment and gatekeep referralsfor older adult inpatient beds and facilitate early discharge from hospital.The HTT's are a multidisciplinary service providing care to residents primarily over the age of 65.

Education and Qualtifcation Essential
Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse (2) Evidence of continuing professional development A certificate in Practice Supervisor Training, Practice Assessor Training, Practice Supervisor or Assessor Training, SSSA-aligned training, or a recognised Mentorship Certificate (or equivalent qualification)"
