We are looking for an enthusiastic, experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly and supportive General Practice team.
This is an excellent opportunity for an ANP who enjoys working autonomously as part of a multidisciplinary primary care team and is committed to providing high-quality, patient-centred care.
Main duties of the job
We are ideally looking for an Independent Prescriber who is confident in assessing, diagnosing and managing patients presenting with a wide range of conditions commonly seen within General Practice.
A key part of the role will be acting as the main clinical point of contact for one of our care homes, providing continuity of clinical care for residents and developing a strong and effective working relationship with the care home team. This will include undertaking a scheduled weekly ward round and working closely with our GPs and wider multidisciplinary team to support the health and wellbeing of care home residents.

Clayton Brook Surgery have an excellent cohesive and dedicated team including; 2 GP Partners, Salaried GPs, Practice Manager, Operations Manager, Finance Manager and a strong administrative team. Our GP Partners are trainers with an emphasis on training GPs of the future.
The permanent role will be a Part-time post of between 16 and 24 hours per week.
The post holder will be flexible in their approach and work closely in collaboration with staff from Moss Side Medical Centre in Leyland.

The Role
The successful candidate will be expected to
Undertake autonomous clinical assessment of patients presenting with a wide range of acute and ongoing health problems
Take comprehensive patient histories and undertake appropriate clinical examinations
Formulate differential diagnoses and develop appropriate treatment and management plans
Request and interpret investigations where appropriate
Prescribe medication within their competence and scope of practice, where appropriately qualified
Provide advice, treatment, health promotion and preventative care
Recognise and appropriately manage clinical risk, including identifying patients requiring escalation or referral
Make appropriate referrals to other services and healthcare professionals
Support the assessment and management of patients with long-term conditions
Maintain accurate, timely and comprehensive clinical records
Work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team
Contribute to the continued development and delivery of high-quality services within the Practice
Care Home Responsibility
A key element of the position will be responsibility for supporting one of the Practice's care homes. The successful candidate will:
Act as the main clinical point of contact for the care home, providing continuity of care and developing an effective working relationship with the care home team
Undertake a scheduled weekly ward round at the care home, reviewing residents and responding to clinical concerns raised by the care home team
Provide appropriate clinical assessment, review and management of residents with acute and ongoing health needs
Support proactive care and ongoing monitoring of residents, helping to identify changes in health needs at an early stage
Request and interpret appropriate investigations
Prescribe medication where appropriately qualified and within their professional competence and scope of practice
Make appropriate referrals and escalate clinical concerns to a GP or other healthcare professional where required
Liaise and work collaboratively with care home staff, GPs, families and members of the wider multidisciplinary team to support coordinated, safe and effective care
Maintain accurate and timely clinical records following care home consultations and reviews
Person Specification
Qualifications Essential
Registered Nurse with appropriate Advanced Nurse Practitioner/Advanced Clinical Practice qualification Previous General Practice or Primary Care experience Experience of autonomous clinical assessment and decision-making Ability to assess and manage patients presenting with acute and ongoing health conditions Excellent communication and interpersonal skills Ability to work autonomously and as part of a multidisciplinary team Ability to work within professional competence and scope of practice Ability to establish an effective working relationship with the allocated care home and undertake the weekly ward round
Desirable
Independent Prescribing qualification Experience of using EMIS or Systmone Previous experience of providing clinical care to care home residents
