An exciting opportunity has arisen for a qualified and experienced Advanced Clinical Practitioner to join our team
You will be expected to function independently and within a clinical team and will demonstrate advanced autonomous clinical practice working across traditional boundaries and augmenting the medical role, utilising assessment, decision making and diagnostic skills. Providing expert advice and to ensure the needs of the patient are met by leading, challenging, and changing practice within the clinical setting.
You will be expected to take the lead and participate in educational issues; clinical supervision; research & audit; evidence-based practice and provide mentorship to other clinical staff working in the field of General Practice.
The role of the ACP will include working collaboratively with other health care professionals in primary and secondary care to ensure a high quality easily accessible service and act as a ACP in the service provided.
The post holder will accept new and follow-up patient referrals within the General Practice, undertaking clinical audit and research, recognising, and acting as an advocate for patients, carers within the service and organisation and leading on changes as required.
Main duties of the job
Gather, analyse and report a wide range of data and information. Acts as a role model and an expert resource for patients and staff throughout the Trust Supports the Consultants, Intensive care /Anaesthetic Registrars and other Doctors, in providing patients with treatment plans within the specialty Performs invasive and non-invasive and complex specialist procedures relevant to the and makes recommendations for treatment. Participates in service development. Writes protocols, guidelines and procedures for own remit which, impacts on other members of multi-disciplinary team. CLINICAL & PROFESSIONAL RESPONSIBILITIES
To be professionally and legally responsible and accountable for all aspects of own work, ensuring a high standard of clinical care is delivered. To act as the patients advocate at all times. To ensure appropriate supervision and clinical support is sought when required and always work within own scope of professional practice and in accordance with the level of developing competence. To have the skills and knowledge to accept clinical responsibility for a diverse and often complex caseload of patients, and to develop the ability to organise this efficiently and effectively with regards to clinical priorities and use of time. To prescribe medications under supervision from an agreed formulary and ensure appropriate medication is prescribed by means of transcribing. Under appropriate supervision, to receive referrals from and make referrals to consultants, GPs and other healthcare professions, review the patient and act accordingly To actively promote a clinical environment that is conducive to high quality patient care and high standards of service delivery Uses specialist knowledge, leadership and expertise to assess, plan and implement care interventions for patients requiring complex care and monitoring which may include organ support and autonomously discharge patients within scope of practice Uses expertise to recommend adjustments/amendments to treatment plans, in consultation with patients to members of the multi professional team The Advanced Practitioner is accountable for own professional actions and not directly supervised. Identify own development needs and set own personal development objectives in discussion with his/her supervisor and line manager, keeping abreast of any new trends and developments and incorporate them as necessary into own practice Responsible for the delivery of specialist training and development of other Health Care Workers within the Trust.
