We are looking for an enthusiastic and committed Practice Nurse to join our friendly and supportive nursing team.

About the role:This is a 30-hour per week position working across our three practice sites in Torquay.
The successful candidate will undertake a varied Practice Nurse role, working alongside our GPs, Lead Practice Nurses and wider clinical team to provide high-quality, patient-centred care.
Chronic disease management is an important part of the role, with particular opportunities for someone with an interest or experience in diabetes and/or respiratory disease.
Main duties of the job
Depending on your existing skills and experience, the role may include:
Chronic disease reviews, including diabetes, asthma, COPD, cardiovascular disease, CKD and hypertension Cervical screening Childhood and adult immunisations Travel health and vaccination Wound care and leg ulcer management Phlebotomy and INR testing ECG and other routine clinical observations NHS Health Checks and cardiovascular risk assessment Health promotion, lifestyle and weight-management advice New patient health checks Contraception and HRT reviews Mental Health and Learning Disability annual reviews Ear irrigation, following appropriate training Assisting with minor surgery and clinical procedures Telephone and face-to-face clinical advice Participation in seasonal vaccination clinics Student supervision and teaching, where appropriately qualified

What we are looking for
We are looking for a registered nurse who is:
NMC registered Able to work autonomously while recognising when to seek advice and support Confident in assessing patients and managing their own clinical workload Experienced in, or keen to develop skills in, chronic disease management A good communicator who is able to build positive relationships with patients and colleagues Accurate and confident using electronic clinical record systems Proactive, organised and able to use their own initiative A genuine team player who enjoys working collaboratively Committed to continuing professional development and delivering high standards of patient carePrevious Practice Nursing experience would be welcomed, but we would also be interested in hearing from nurses with relevant community or hospital experience who are looking to develop their career in general practice.
Additional qualifications in areas such as chronic disease management, cervical cytology, immunisations, travel health, diabetes, respiratory disease or wound care are desirable, but we will consider the overall skills and experience of each applicant.

Diploma / Degree in practice nursing Asthma, COPD and or Diabetes chronic disease management training
Additional Essential
Able to move between practice sites as required Undertake mandatory training annually Full driving licence, personal card and appropriate insurance
