Job summary
The Hepatology Clinical Nurse Specialist (CNS) will play an integral role in the development of a quality driven service to all patients at/or admitted to THHFT with Liver disease. The CNS has a leadership role within the Trust and is responsible for the development of innovative nursing practice within Hepatology. The post holder is expected to facilitate the development of other nurses to benefit patient care and to be an educational resource to all members of the Trust wide multi-disciplinary MDT regarding practice provision of care. The CNS will help to promote and co-ordinate a seamless pathway of care for patients with liver disease, from diagnosis through to treatment and outpatient management.
The post would be Band 6/7 dependent upon experience. For Band 6 applicants, Band 7 would be applied when the post holder undertakes clinics independently, with supervision.
Main duties of the job
The role will involve independently screening patients for liver fibrosis using Fibroscan (training provided), carrying out comprehensive face to face and telephone assessment of patients and providing health education to patients and their families/ caregivers through supervised, nurse led clinics. The post holder will manage a caseload of a range of patients with liver disease and will be expected to work independently within the remit of their role, education, support and supervision will be provided. The CNS will be required to liaise closely with members of the MDT to progress patient care and maintain safety at all times. High quality IT skills are essential for this role to assist with data collection, audit, analysis and potential opportunity to present data at regional and national meetings.
