The Virtual Ward is an innovative service and the aim to is support people to receive care, support and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.
The successful applicant will provide clinical leadership and line management to the Virtual Services Team. The aim of Virtual Ward is to support people to receive care, support and interventions they need in their own home which will lead to improved quality of life and outcomes for people as well as reducing hospital length of stay and unavoidable hospital admissions. They are expected to lead by example, motivate and empower team members to achieve their potential and provide a visible, accessible and authoritative presence in the Virtual Services Team to whom staff, patients and their families can turn to for assistance advice and support in relation to their hospital discharge and care needs.
Main duties of the job
The post holder will provide clinical leadership and line management to the Virtual Services Team. The team manager will:
Lead by example, motivate and empower team members to achieve their potential
Provide clinical leadership and line management to the staff
Provide a visible, accessible and authoritative presence in the Virtual Services Team to whom staff, patients and their families can turn to for assistance advice and support in relation to their hospital discharge and care needs.
Create and maintain a learning environment that ensures that the educational professional and practise development needs of the staff are met
Work Trust wide to promote the safe and effective utilisation of virtual care pathways.
The role entails working across organisations and links with social services and Primary Care Teams and is key in working with family and carers to support the safe community based care.
