We are looking for a motivated, compassionate, and enthusiastic Community Support Practitioner Worker to join our Urgent Community Response and Intermediate Care Team across Scarborough and Ryedale Community Services.
This is a rewarding opportunity to make a real difference to patients' lives, working alongside a multidisciplinary team of Physiotherapists, Occupational Therapists, and Nurses. You will play an important role in supporting rehabilitation programmes and functional activities, helping individuals regain confidence, maximise their recovery, and remain independent within their own communities.
You will work with people experiencing a wide range of medical conditions and pathologies, providing support during a period of rehabilitation aimed at improving mobility, functional ability, and independence.
In return, you will receive regular supervision, access to ongoing learning and development opportunities, and a comprehensive induction programme designed to help you feel supported and succeed in your role.
Main duties of the job
Day to day, the role includes the following care carried out in the patient's own home with supervision/delegation oftherapists/registered nurses:
Patient care following a fall Moving & handling of patients Vital observations End of life care and urgent support for people living with frailty Recognise and escalate issues of concern and/or safety Assisting with activities of daily living, e.g., personal care tasks, meal preparation following injury/illness to prevent hospital admission- supporting and monitoring mobility, and rehabilitation programmes which include aspects of nursing, occupational Therapy and Physiotherapy. Skills and Competencies Essential
Able to demonstrate excellent inter-personal communication skills Able to plan and prioritise effectively and efficiently
Experience Essential

Experience of working within a healthcare environment or service
