Job summary
Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region.Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve, and we are looking for highly motivated Wellbeing Practitioner to join our Wellbeing Service team.
Based in Mid Essex, the successful candidate will join the extensive MDT and will undertake holistic assessments for patients and family members, working with them to provide practical and emotional support. This essential support may be delivered face to face in peoples homes or the hospice, in groups or virtually.
Main duties of the job
You will be a caring, compassionate and empathetic individual with proven experience of supporting people experiencing distress. You will have experience of working in health or social care and have excellent communication skills. You will have experience of advocacy and carer needs. You will have the ability to work autonomously as well as part of a multidisciplinary team. You will be a car driver with access to your own vehicle

Job description Job responsibilities
o deliver compassionate, psychologically informed and person-centred wellbeing support to patients, carers and families affected by palliative illness, promoting independence, resilience and self-management through early intervention and holistic wellbeing assessment. Working within a consultation-led model, the Wellbeing Practitioner will take an inclusive approach to understanding individuals' emotional, social, financial, spiritual, intellectual and physical wellbeing needs, strengths, risks and goals. Through collaborative wellbeing planning, the postholder will provide or facilitate access to the most appropriate interventions, services and community resources, ensuring support is responsive to changing needs and circumstances. The role exercises professional judgement to coordinate timely support, empower individuals to achieve their wellbeing goals, and ensure appropriate escalation, signposting or referral where needs fall beyond the scope of the service.

Person Specification
Skills/Ability/Knowledge Essential
Knowledge of Safeguarding children and adults Have a holistic understanding of wellbeing and the impact of life limiting illness on a person, and their family/support systems Ability to adapt to the changing needs of the organisation Ability and experience of working on own initiative and autonomously Ability to work within professional boundaries and recognise limits of competence Willingness to work in partnership with community resources and support services Proven ability to deal with people and situations sensitively and appropriately

Ability and willingness to travel throughout Mid-Essex using own vehicle
Qualifications Essential
NVQ Level 4 [or equivalent] in Health or Social Care, Wellbeing, Community Practice or a related discipline, or relevant skills and experience
Experience Essential
Experience of undertaking holistic wellbeing consultations or assessments exercising analytical and problem-solving skills to determine appropriate interventions Experience of and ability to work independently and as part of an MDT Team Experience communicating effectively with a wide range of audiences, including oral and written communication skills Experience of responding appropriately to safeguarding concerns and risks Experience of working with IT systems Experience of supporting individuals experiencing emotional distress, grief, loss or adjustment Experience of managing a caseload and reviewing interventions in response to changing need
Desirable
Experience in bereavement, palliative care or hospice settings Experience of facilitating groups or workshops using psychoeducational approaches Experience of working with volunteers
