Care Team Nurse (LPN/RN)

As a valued member of our Care Team, you will play an essential role in coordinating patient care, supporting providers, educating patients, and helping individuals manage acute and chronic health conditions. You will be part of a collaborative, multidisciplinary team that places patients at the center of every decision.
What You'll Do
Patient-Centered Nursing & Care Coordination
• Support the ongoing management of patients with stable chronic conditions in collaboration with the Primary Care Provider.
• Coordinate treatment plans, follow-up care, referrals, and patient services.
• Manage patient triage, intake, and monitoring for acute medical conditions.
• Provide telephone nursing advice following established policies and protocols.
• Monitor patients and provide appropriate nursing interventions when the Primary Care Provider is unavailable.
Patient Education & Health Promotion
• Educate patients on self-care, medication management, diabetes management, wound care, inhalers, nebulizers, and self-injections, as appropriate.
• Explain treatments and procedures to help patients understand and participate in their care.
• Obtain patient education materials in languages appropriate to the patient's needs.
• Promote wellness, illness prevention, and effective management of acute and chronic conditions.
Clinical Support & Nursing Procedures
• Assist providers with examinations, procedures, medication administration, and treatments within the scope of professional nursing practice.
• Perform clinical procedures such as EKGs, spirometry, nebulizer treatments, and minor procedures as appropriate.
• Perform waived testing, including blood glucose and HbA1c testing, following established protocols.
• Assist with patient rooming, vital signs, injections as ordered, and clinic workflow.
Teamwork, Quality & Continuous Improvement
• Collaborate throughout the day with Primary Care Providers and Care Team Medical Assistants to coordinate patient care and improve workflow.
• Manage the Care Team inbox, patient communications, referrals, and follow-up activities.
• Review critical and urgent results and bring items requiring action to the Primary Care Provider.
• Support patient safety, infection prevention, regulatory compliance, and continuous improvement in ambulatory care.

What You'll Bring
Required Qualifications:
• Current Oregon Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• Current Basic Life Support (BLS) certification for Healthcare Providers
• Ability to exercise independent judgment and make appropriate clinical decisions.
• Ability to prioritize responsibilities, problem-solve, and manage a high volume of patients in a dynamic healthcare setting.
• Previous clinic experience preferred.
