Job summary
The post holder will be an accountable professional, responsible for delivering high quality, patient centered, compassionate care to patients being discharged via the discharge and transport hub. The post holder will assume responsibility and accountability for patients to meet their needs in a safe caring environment that promotes dignity, privacy and respect. The aim of the discharge lounge is to promote patient flow, improve quality of care and patient experience.The post holder has a duty of care to patients and is expected to comply fully with best practice standards and comply with trust policies for personal and patient safety and the prevention of healthcare associated infections.
To coordinate outstanding tasks to ensure patients are discharged safely and timely from hospital and to provide support to the multi professional team by undertaking appropriate duties.
To be responsible for the provision of skilled nursing care, within the multidisciplinary care team. There will be a requirement to supervise Health Care Assistants and student nurses on a day to day basis in achieving objectives.
