Job summary
The Fishponds Family Practice is looking for an experienced Advanced Nurse Practitioner and independent prescriber to join our friendly and established multidisciplinary team.
This is an opportunity for an ANP who enjoys the breadth of primary care and wants a role combining minor illness and acute assessment with the management of long-term conditions.
We are particularly keen to hear from clinicians with experience in diabetes care
Is a registered nurse with current NMC registration has completed an advanced clinical practice qualification at Master's level or has equivalent experience
Holds an independent prescribing qualification with appropriate NMC prescribing
Has recent experience of safe, independent prescribing
Has substantial experience assessing and managing minor illness, acute or undifferentiated presentations in primary care or a comparable setting
Is confident in history taking, examination, clinical reasoning and diagnostic decision-making
Can recognise red flags, manage clinical uncertainty and escalate appropriately
Has experience of long-term-condition management, including review, treatment planning and follow-up
Works well as part of a multidisciplinary team while being comfortable making clinical decisions
Experience and additional training in type 2 diabetes management would be particularly valuable. Experience with EMIS Web, QOF, teaching, supervision, audit or quality improvement would also be welcomed.

Main duties of the job
Working as an autonomous practitioner within an agreed scope of practice, you will assess and manage patients presenting with minor illness and appropriate undifferentiated symptoms.
You will take histories, undertake clinical examinations, develop differential diagnoses, request and interpret investigations, prescribe independently, agree treatment and follow-up plans, and refer or escalate patients where appropriate.
Alongside acute work, the post will include planned long-term-condition care. Depending on your experience and interests, this may include structured reviews, medicines optimisation, risk assessment and support for patients managing conditions such as diabetes, cardiovascular or respiratory disease.
The precise balance of acute and planned care will be agreed with the successful candidate, taking account of their experience, competencies and development interests.

Fishponds Family Practice is a long-established teaching practice based on Beechwood Road in Fishponds, Bristol, caring for more than 12,500 patients.
Our multidisciplinary clinical team includes GP partners and salaried GPs, nurses, healthcare assistants and pharmacy colleagues, supported by an experienced Care Coordinator team.

Job description Job responsibilities
Provide advanced, person-centred clinical care for registered patients, with a planned mix of minor illness and acute assessment alongside long-term-condition management.
Work as an autonomous nurse prescriber within a documented scope of practice, using the wider clinical team for discussion, referral and escalation. The exact caseload, age range, home visiting contribution and diabetes responsibilities will be agreed according to competence and service need.
Clinical assessment and minor illness
Assess patients presenting with minor illness and appropriate undifferentiated symptoms through face-to-face or telephone consultations as agreed. Take histories, examine, form differential diagnoses and identify red flags, deterioration and emergencies.
Request and interpret appropriate tests, agree evidence based treatment and safety netting, arrange follow-up, and refer or escalate to GPs, urgent care, community services or secondary care as needed.
Prescribe independently within competence, current legislation and practice policy. Review treatment effectiveness, adverse effects and medicine safety.
Take responsibility for results, prescriptions, correspondence, referrals and other clinical administration generated by own consultations, ensuring clear handover when absent.
Long-term conditions and diabetes
Deliver structured reviews and care planning for long term conditions within agreed competencies.
Contribute to diabetes reviews and follow-up where competent. Assess cardiovascular and other risks, interpret monitoring results, review treatment and identify complications or need for specialist input.Experience with type 2 diabetes is especially valuable.
Work with the nursing, GP and pharmacy teams to coordinate recalls, follow-up and continuity, and support relevant QOF and local service requirements without treating targets as a substitute for individual clinical need.
Teamwork and quality
Work constructively with GPs, nurses, paramedics, pharmacy staff, care coordinators and community colleagues.
Contribute to clinical meetings, case discussion, supervision or teaching appropriate to experience, and development of evidence based pathways.
Participate in audit, incident learning and quality improvement, including review of prescribing and long term condition care.
Professional and organisational responsibilities
Maintain NMC registration, prescribing annotation, revalidation, mandatory training and continuing professional development. Participate in appraisal and clinical supervision.
Maintain accurate, timely clinical records and coding in EMIS. Use practice systems safely and protect confidential information.
Apply safeguarding, consent, capacity, infection prevention and control, information governance, equality and health and safety requirements.
Recognise the limits of personal competence, maintain an agreed scope of practice and escalate concerns promptly.
This description will be reviewed with the postholder as the role and service develop. Changes to clinical scope will require suitable competence, support and agreement.
Person Specification
Qualifications Essential
Registered nurse with current NMC registration. Completed advanced clinical practice qualification at masters level or equivalent experience. Completed independent prescribing qualification with NMC prescribing annotation and demonstrable safe, recent independent prescribing practice. Substantial recent experience independently assessing and managing minor illness and acute, undifferentiated presentations in general practice or a comparable primary care setting. Experience of long term condition management, including structured review, treatment planning and follow up. Advanced history taking, examination, clinical reasoning and diagnostic decision making; ability to identify red flags and work safely with uncertainty. Knowledge of evidence-based guidance, prescribing safety, safeguarding and clinical governance. Ability to communicate sensitively, maintain clear records, manage workload and work effectively in a multidisciplinary team.
Desirable
Experience and relevant training in diabetes care, particularly type 2 diabetes reviews, treatment adjustment and complication prevention in primary care. Experience using EMIS, and knowledge of QOF and local primary care pathways. Experience of teaching, clinical supervision, audit or service improvement. Experience managing other long term conditions, such as cardiovascular or respiratory disease, within evidenced competence.
