Job summary
We are looking for a dedicated and forward-thinking Nurse to join our friendly and supportive General Practice team.
This is an exciting opportunity for a qualified nurse to play a key role in delivering high-quality, patient-centred care, with the opportunity to use and develop your clinical skills across a varied and rewarding general practice environment.
Working within your professional scope of practice, you will take an active role throughout the patient journey from assessment and history taking through to clinical decision-making, treatment, care planning and evaluation. You will be encouraged to use your clinical expertise, critical thinking and professional judgement to provide safe, effective and compassionate care.
You will work closely with GPs, nurses, healthcare assistants, administrative colleagues and other members of our multidisciplinary team, contributing to a collaborative environment where teamwork, innovation and continuous improvement are valued.
The role also provides opportunities to contribute to the ongoing development of our services, support practice policies and initiatives, and provide clinical leadership within your area of expertise.

Main duties of the job
The successful candidate will work as part of our friendly and supportive practice team, providing safe, effective and patient-centred nursing care. Main duties will include:
Providing high-quality nursing care within your professional scope of practice.Carrying out wound care and dressings.Administering injections and vaccinations.Taking blood samples and carrying out urine testing/dipstick analysis.Recording routine clinical observations, including blood pressure, height and weight.Childhood ImmunisationsECG'sEar SyringingCervical SmearsChronic Disease ClinicsSupporting the wider clinical team with patient assessments, treatment and ongoing care.Working collaboratively as part of the multidisciplinary practice team.Maintaining accurate and timely patient records and completing associated clinical administration.Providing advice and support to patients where appropriate, promoting health and wellbeing.Undertaking additional clinical duties as required by the practice.Whilst some of the above is mandatory to apply, training and support will be provided to develop skills in areas such as childhood immunisations, spirometry, ECGs, ear syringing, cervical screening and chronic disease management clinics. We are a friendly and supportive practice with a multidisciplinary team of approximately 35 staff, including GPs, nurses, healthcare professionals, administrative and reception colleagues, supported by an experienced management team. We cross-cover three different sites; Trethomas, Machen & Llanbradach.

Job description Job responsibilities 1. Clinical Practice
The Practice Nurse will provide safe, effective and patient-centred care within their professional scope of practice, including:
Carrying out clinical assessments and developing appropriate care plans.Providing travel health advice, including vaccinations, medication, safe sex, food and water hygiene and sun protection.Delivering childhood and adult vaccinations in accordance with current national guidance and relevant Patient Group Directions (PGDs).Supporting health promotion, including advice on healthy eating, physical activity, smoking cessation and alcohol awareness.Undertaking ear irrigation and cervical screening in accordance with current guidance and professional standards.Performing venepuncture and obtaining blood samples in line with practice procedures.Assessing and managing wounds using evidence-based care.Participating in chronic disease management clinics and supporting patients with long-term conditions.Undertaking ECGs, suture removal, urine testing and other routine clinical procedures.Collecting, handling, storing and dispatching clinical specimens appropriately.Recognising and responding to anaphylaxis and other clinical emergencies in line with current UK guidance.Performing CPR and responding appropriately to medical emergencies.Supporting minor surgery and family planning services, including contraceptive procedures, where required and within competence.Undertaking anticoagulation/Warfarin monitoring and providing appropriate patient assessment and advice.Obtaining and accurately documenting informed consent.Maintaining high standards of infection prevention and control.Working collaboratively with the multidisciplinary team to provide coordinated patient care.2.
