Job summary
We are employed by Community but based on the acute hospital site at NDDH. Our remit is to assess and procure onward care for patients being discharged from hospital on discharge pathways 2 and 3, however, on occasions we may also support with pathway 1.
The postholder will be expected to complete comprehensive, holistic assessments, provide expert support to ward staff and resolve discharge delays, working closely with other relevant acute and community Health and Social Care teams to ensure the person is then discharged safely and efficiently.

An exciting opportunity has arisen for a Nurse to join our Care Transfer Hub at Royal Devon University Healthcare NHS Foundation Trust, North Devon District Hospital.
The Care Transfer Hub is an integrated multidisciplinary team bringing together Complex Discharge, Adult Social Care and Single Point of Access services.
Applications are invited from experienced Registered Nurses with a passion for patient advocacy, problem solving and patient flow. A background in both Community and Acute services would be an advantage.
Knowledge of health and social care funding and eligibility criteria would be beneficial but not essential.
Whilst the role can be challenging at times, you will be well supported by a friendly team of Health and Social Care professionals. You will have the opportunity to gain valuable transferrable skills, in this unique team which works across acute, community and the private provider settings. We try to ensure a good work life balance for all of our members and feel that we are approachable and seek to encourage and support our team members to develop in their roles.
Full time hours would be preferred but we can be flexible for the right candidate.
