Job summary
We are looking for an enthusiastic and compassionate Bank Staff Nurse to join our Living Well Services team, based at our Rowans Living Well Centre in Purbrook.
The Living Well Services provide support to people living with a life-shortening or incurable illness, often at a much earlier stage in their illness journey. Our aim is to help people maintain their independence, wellbeing and quality of life, while supporting their families and carers.
Working alongside the Living Well Services Team, you will provide nursing support across our Living Well Centre and Community Satellite Hubs. You will play an important role in assessing individual needs, coordinating appropriate care and therapies, and helping patients and their families remain informed and in control of decisions about their care and future.

Main duties of the job
Providing nursing support within the Living Well Services, Monday to Friday, 8:30am4:30pm, including our Community Satellite Hubs. Supporting people who are seeking help and support before they reach the final stages of their life. Providing opportunities to inform, support, care for and re-able people living with a life-shortening or incurable illness, as well as those approaching the end of their natural life. Providing a welcoming environment that offers companionship and support to patients and their carers. Promoting health, wellbeing and independence within the local community. Supporting patients with future planning and decision-making, helping them to remain in control of their lives for as long as possible. Assessing and triaging patients and carers and arranging appropriate programmes of care and therapy to meet their individual needs. Using assessment tools including IPOS and CSNAT to identify needs and refer patients to appropriate professionals and therapies both within and outside the hospice. Working collaboratively with colleagues and external professionals to ensure patients and carers receive appropriate, coordinated support.

Job Purpose
To contribute to the delivery of the Living Well Service, operating Monday to Friday, 9.00am4.00pm, with extended hours until 6.00pm on Wednesdays, including Community Satellite Hubs.Support the development and delivery of services that respond to the needs of people seeking support from Rowans Hospice prior to the final stages of life.Provide opportunities to inform, support, care for and enable people, their families and friends who are affected by a life-shortening or incurable illness, or who are approaching the end of their natural lifespan.Promote an environment that provides companionship, support and meaningful engagement for patients and carers.Support the health and wellbeing of the local community and, where appropriate, enable individuals to engage in future planning and decision-making, supporting them to remain in control of their lives for as long as possible.Ensure patients and carers are appropriately assessed and triaged, with individual programmes of care, support and therapies arranged according to their identified needs.Assess the needs of patients and carers using appropriate assessment tools, including IPOS and CSNAT, and identify and facilitate referrals to relevant professionals and therapy services both within and outside the hospice.Liaise effectively with members of the clinical team, Hospice Companions Coordinator, Volunteer Services Manager, Primary Care and Social Care professionals to facilitate continuity of care within the community.Liaise with relevant inpatient and outpatient hospital departments to support the coordination and continuity of patient care.In conjunction with the LWS Manager, contribute to the development and expansion of Living Well Services, including increasing access to therapy sessions and providing flexible opportunities throughout the week.Promote a therapeutic environment by utilising the skills and expertise of therapists and artists working within the service and actively contributing to relevant projects and activities, including mindfulness.Provide appropriate advice regarding symptom control and care management to Living Well Service patients and, where appropriate and within the scope of the role, to other healthcare professionals.Report relevant information regarding patients' conditions, care or treatment to the LWS Manager, Deputy LWS Manager, Hospice Doctor or Palliative Care Hub, as appropriate.Report any matters affecting the effective and smooth running of the centre or Community Satellite Hubs to the LWS Manager, Deputy LWS Manager or Director of Clinical Services, as appropriate.Report complaints and safeguarding concerns raised by patients, relatives or individuals acting on their behalf to the appropriate manager and support the investigation of complaints where required, working within the boundaries and responsibilities of the role.Maintain strict confidentiality regarding patients and carers and ensure information is only shared appropriately with professionals where this is necessary to support the individual's care, wellbeing or protection.Demonstrate a consistently high standard of courtesy, dignity and consideration towards patients, carers, relatives and visitors.Promote and maintain a high quality of care by adhering to relevant standards, policies, procedures and best practice.

Experience Essential

Qualifications Essential
RGN / Registered Nurse qualification Current registration with the relevant professional regulatory body A relevant post-qualifying qualification
