An exciting new opportunity has arisen for an Advanced Clinical Practitioner to join our Integrated Community Specialist Palliative Care Team (ICSPC)
In an exciting new development for our community services LOROS Hospice is seeking an experienced Advanced Clinical Practitioner to join our ICSPC Team providing expert, compassionate care for people with complex palliative and end of life care needs. This new role is aimed at increasing the responsiveness of our community team across Leicester, Leicestershire and Rutland.
Main duties of the job
Working alongside palliative medicine consultants, neighbourhood teams and wider health and social care partners, you will practice autonomously at an advanced level, undertaking assessment, diagnosis, prescribing, care planning and clinical management. You will play a key role in supporting people to remain in, or return to, their preferred place of care, helping to prevent avoidable hospital admissions and support timely discharge.
You will also provide visible clinical leadership, expert advice, supervision and education across hospice and community services, whilst contributing to audit, research and evidence-based service improvement.
We are looking for a registered healthcare professional with advanced clinical skills, strong decision-making ability and experience in palliative, end of life, cancer or complex community care. You will be compassionate, confident working autonomously and committed to delivering high quality, person-centred care across organisational boundaries.

This role is aligned to the four pillars of Advanced Clinical Practice: Clinical Practice, Leadership and Management, Education and Research.
The post holder will practice autonomously at an advanced level, providing expert assessment, diagnosis, prescribing and management for people with complex palliative and end of life care needs as part of the Integrated Community Specialist Palliative Care Service (ICSPC). They will offer visible clinical leadership alongside our Palliative Medicine Consultants across community services, support education, supervision and development of colleagues and contribute to audit, research, evaluation and evidence-based service improvement.
This role will enhance our community offer by extending the clinical skills of the non-medical team and increase the responsiveness of our same day offer. Manage people with complex palliative care needs in a range of community health settings to avoid unnecessary acute admissions, support timely discharge, and enabling people to achieve their Preferred Place of Care.
The post holder will facilitate and support excellence in clinical practice as part of the ICSPC by collaborating with neighbourhood teams and working across organisational boundaries. Demonstrate the organisational Values & Behaviours and show responsible use / expenditure of LOROSs resources.

Clinical skills and abilities Essential
Advanced clinical assessment. Clinical decision making. Symptom management. Risk assessment - able to identify, assess and manage clinical risk. Diagnostics -able to appropriately use clinical investigations and diagnostic information to support assessment and management while recognising the limitations of investigations in advanced illness. Qualifications Essential
Relevant Professional Qualification. Registered General Nurse or Paramedic. Primary degree. Master's degree in Advanced Practice and or Palliative Care. Non-medical Prescriber V300. Clinical Examination and Diagnostics modules if not part of Masters qualifications.

Experience Essential
Extensive post qualification experience Portfolio of evidence showing achievements in practice speciality Supervision experience and the ability to support trainee practitioners in their development and career progression. Ability to work autonomously at advanced practice level and make expert clinical decisions to support the development of services and inform practice. Ability and evidence to lead on related research and audit in relation to the development of advanced practice. Communication of difficult messages to patients and families. Evidence of multi-agency or professional working Negotiation and conflict management skills within the context of multi-agency working. Demonstrates efficient experience in leadership of team/s in the context of multi-agency working
Desirable
