About this role

Job summary

Queen Square Medical Practice is seeking an experienced and motivated Practice Nurse with a particular interest and expertise in diabetes management to join our friendly and progressive clinical team.

This is an exciting opportunity for a skilled nurse looking to participate within a leading role in the delivery and development of high quality diabetes and long term condition services within a well established and patient focused practice. The successful candidate will have substantial experience in general practice nursing and be confident managing patients with diabetes and other chronic diseases. Independent prescribing would be advantageous but is not essential.

Main duties of the job

The successful candidate will participate in leading and supporting the delivery of diabetes care within the practice, ensuring patients receive evidence based assessment, treatment, education and ongoing management.

Key Responsibilities:

Diabetes Management Participate in leading diabetes clinics within the practice. Undertake comprehensive diabetes reviews, including annual assessments. Support achievement of QoF and other quality indicators relating to diabetes care. Interpret and act upon investigation results. Optimise diabetes management in line with current NICE guidance. Provide insulin initiation and titration support where appropriately qualified. Deliver patient education to promote self-management and improve health outcomes. Identify and proactively manage patients at risk of complications. Long-Term Condition Management Manage patients with chronic conditions including: Diabetes Hypertension Cardiovascular disease COPD Asthma CKD Undertake clinical assessments and formulate management plans. Monitor treatment effectiveness and escalate concerns where appropriate.

Queen Square Medical Practice is a busy independent GP Practice based in Lancaster with a patient list of15500. We have 9 GP Partners and 5 Associate GP's who are supported by ANPs, paramedics, Pharmacists, Nurses, HCA's and Admin team. We also have a branch site in Caton.

To provide high quality nursing care within the practice, with a particular focus on the management of patients with diabetes and other long term conditions. The post holder will work as an autonomous practitioner within their scope of practice, supporting the delivery of safe, effective and patient centred care whilst contributing to the ongoing development of nursing services.

Cervical cytology. Immunisations and vaccinations. Travel health advice and vaccinations. Wound care and dressings. Health promotion and lifestyle advice. ECG recording. NHS Health Checks. Infection prevention and control activities. Clinical governance:

Maintain accurate and contemporaneous patient records. Participate in audits and quality improvement initiatives. Contribute to significant event reviews and organisational learning. Ensure compliance with professional and regulatory standards. Team working:

Work collaboratively with GPs, pharmacists, HCAs and wider multidisciplinary teams. Support mentoring and development of junior members of the nursing team. Contribute to service development and innovation. Participate in team meetings and training activities. Professional responsibilities:

Maintain NMC registration. Participate in annual appraisal and personal development planning. Undertake mandatory training. Maintain professional competence and evidence of CPD. Work within NMC Code and practice policies. Confidentiality:

Person Specification

Experience Essential

Significant experience working as a Practice Nurse. Experience of leading diabetes reviews and management. Long-term condition management experience. Cervical cytology. Vaccination and immunisation experience. Experience working with clinical IT systems. Insulin initiation and management. Experience leading or supporting long term condition services.Experience undertaking audits and quality improvement projects.Experience of supervising junior staff.

Qualifications Essential

Registered General Nurse RGN. Current NMC registration. Recognised qualification in diabetes management. Evidence of ongoing professional development.

Desirable

Independent Nurse Prescriber V300. Diploma/Degree level diabetes qualification. Mentor or teaching qualification.

Knowledge and skills Essential

Knowledge and Skills Strong knowledge of diabetes care and current NICE guidance. Clinical assessment and consultation skills. Excellent communication skills.
