Main duties of the job
As a Community IV Nurse, you will provide specialist nursing care to patients receiving IV therapies and Parenteral Nutrition in their own homes and community settings. You will support patients throughout their treatment journey, ensuring safe and effective care while promoting independence and confidence in managing their treatment.
This role offers the opportunity to work autonomously while developing strong relationships with patients, carers and healthcare professionals. You will be part of a collaborative multidisciplinary team dedicated to achieving the best possible outcomes for patients.

Key Responsibilities
Deliver safe, high-quality, patient-centred care in patients' homes and community settings.
Administer prescribed Parenteral Nutrition (PN), IV therapies and clinical interventions in line with SOPs and professional standards.
Assess patients, identify changes in condition and escalate clinical concerns appropriately.
Support treatment initiation, ongoing care and transitions from hospital to home.
Train, educate and empower patients, families and carers to safely manage treatment and build independence.
Provide clinical advice and support while building strong relationships with patients and healthcare professionals.
Work collaboratively with NHS colleagues, Senior Homecare Nurses, Clinical Nurse Managers and wider teams to ensure continuity of care.
Maintain accurate clinical records and work in line with Clinical Governance, NMC standards and Inizio policies.
Support patient safety, quality improvement, service development and achievement of programme KPIs.

Person Specification
Qualifications Essential
Essential Current NMC Registration. Degree-level qualification in Nursing or equivalent experience. Evidence of continuing professional development. Current valid UK driving licence.
Desirable
Desirable Additional qualifications or training relating to IV therapies, vascular access or Parenteral Nutrition. Relevant clinical certifications associated with community nursing or specialist therapies.
Experience Essential
Essential Minimum one year post-registration nursing experience. Experience delivering high-quality patient care in a clinical setting. Experience working independently and managing a varied workload. Experience building effective relationships with patients, carers and healthcare professionals.
Desirable
Desirable Experience in CVAD and/or Parenteral Nutrition services. Experience delivering patient education and support programmes. Experience working within community, homecare, acute or NHS healthcare settings. Pharmaceutical or healthcare industry experience.
