Position Details:
Manager - Home Health and Hospice Intake 1630 Des Peres Rd, 63131
Monday - Friday 8a-4:30p
• On call rotation
Responsible for leading and coordinating Hospice and Home Health Intake operations across Mercy Ministries, ensuring a consistent, efficient, and high-quality experience for patients, families, referral partners, and caregivers. This role will initially oversee Hospice Intake for the East region and serve as a key leader in the strategic alignment and integration of Intake services across additional Hospice communities, with future expansion to include Home Health Intake. The leader will drive process standardization, support caregiver engagement and development, optimize resource utilization, and monitor operational, quality, and service metrics. Works collaboratively with leaders, physicians, referral sources, and caregivers across Hospice, Home Health, and the broader health system to improve communication, streamline workflows, and support continued growth and excellence in post-acute care services
Qualifications:
• Required Education: • Graduate from a school of nursing is required.
• Preferred Education: • Bachelor’s degree in nursing is preferred.
• Experience: • 3 year of nursing experience.
• Preferred Experience: • Previous leadership/management experience.
• Licensure: • Active RN (Registered Nurse) license in state of practice is required.
• Certifications: • BLS Certification required within 90 days of hire.
• Preferred Certifications: • Certified Hospice and Palliative Nurse (CHPN) certification.
• Other: • Demonstrates skill in human relations and communications is required. Must have broad understanding of the patient care process and be able to demonstrate clinical expertise in their area of responsibility.
