We are seeking a Practice Nurse to join our dedicated team and help deliver high-quality, patient-centred care to our local community. This maternity cover position offers an excellent opportunity to work within a supportive and collaborative environment, with opportunities for ongoing professional development.
Previous experience in general practice and chronic disease management would be advantageous, but is not essential, as training and support will be provided for the right candidate.
This is a maternity leave cover post, with the possibility of continuing in a part-time role following the end of the fixed-term contract.

Main duties of the job

The successful candidate will be responsible for:
Delivering a range of general nursing services, including: Chronic disease management Immunisations and vaccinations Wound care Cervical screening Health promotion and lifestyle advice Working collaboratively with GPs, Advanced Clinical Practitioners, Healthcare Assistants and the wider practice team to provide holistic patient care. Supporting the achievement of Quality and Outcomes Framework (QOF) targets and the management of patients with long-term conditions. Maintaining accurate clinical records and adhering to practice policies and professional standards.

Job Summary
The post holder is responsible for the delivery of general practice nursing to the whole practice population.
The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. To work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures.
Key Responsibilities
Clinical practice
Provide assessment, screening treatment, services and health education advice
Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears, etc.
Assist with the organisation and co-organisation of the provision of nursing services for the practice
Provide general nursing treatment to patients in participation with general practitioners using agreed protocols
Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition to include health promotion and well being
Be competent and confident in managing people with long term health conditions
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug based treatment methods using a management plan, and in line with national and local policies and practice needs
Work with patients in order to support compliance with and adherence to prescribed treatments
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
Support and manage health needs of women presenting for family planning and cervical cytology consultations
Implement and participate competently in vaccination and immunisation programmes for both adults and children
Advise, support and administer vaccinations competently where appropriate for patient travelling abroad
Promote and deliver evidence-based care for patients presenting with aural conditions
Assessing and treat routine wounds including trauma
Assessing to support good wound management in complex situations (e.g. use of Doppler technique)
Assisting the general practitioner and/or undertaking some simple procedures
Maintain the stock
Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Act as a chaperone when required following the practice chaperone policy
To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager
Person Specification
Qualifications Essential
Essential Registered Nurse with current NMC registration. Excellent communication and interpersonal skills. Able to work effectively both independently and as part of a multidisciplinary team. Committed to delivering high-quality, patient-centred care.
Desirable
Desirable Experience working within primary care/general practice. Experience managing patients with long-term conditions. Cervical cytology Immunisation Experience Training and development will be supported for the right candidate.
