An exciting opportunity has arisen for an experienced ACP to join our Home Visiting Team, working in and around Boston. You will be an enthusiastic and motivated ACP who wishes to influence patient care in this bespoke area of primary care.
Main duties of the job
Your main duties will be to carry our urgent on the day home visits, including visit triage, seeing on the day patients for acute and minor illness, for housebound patients and care home residents, working on behalf of Boston Primary Care Network (PCN) practices.

An exciting opportunity has arisen for an experienced ACP to join our Home Visiting Wellbeing Team, working in and around Boston. You will be an enthusiastic and motivated ACP who wishes to influence patient care in this bespoke area of primary care.
Your main duties will be to assist daily with home visits including visit triage, seeing on the day patients for acute and minor illness, for housebound and care home patients, working on behalf of Boston Primary Care Network (PCN) practices.
You will be employed and supported by Boston PCN Ltd and will work with a diverse, multi-skilled, experienced professional team of clinical and non-clinical, friendly and committed staff.
Boston PCN covers six practices across Boston, Kirton and Swineshead areas.
Person Specification
Knowledge and skills Essential
Advanced history-taking, physical assessment and clinical examination skills. Ability to recognise and respond to red flags, clinical risk and deterioration. Safe prescribing and medicines management knowledge appropriate to the role. Knowledge of safeguarding, consent, capacity, confidentiality and information governance. Excellent communication, consultation and shared decision-making skills. Ability to prioritise workload, manage uncertainty and work effectively under pressure. Competent use of clinical IT systems, ideally including SystmOne.
Qualifications Essential
PLEASE SEE JD FOR FULL DETAILS Current registration as a healthcare professional with the relevant regulatory body, for example NMC, HCPC or GPhC. Masters degree in Advanced Clinical Practice, or evidence of working towards an approved programme where the role permits trainee status. Independent prescribing qualification Evidence of advanced clinical assessment and consultation skills. Evidence of continuing professional development relevant to the post Experience of working within a demanding primary care clinical setting, preferably GP practice. Full and clean UK Driving Licence and access to transport.
Desirable
Experience of working in a community health / home visiting care setting linked to GP Practice. Experience of working in a multi-disciplinary health care team.
Experience Essential
Significant post-registration clinical experience in a relevant setting. Experience of autonomous assessment, clinical decision-making and management planning. Experience of managing acute, chronic, complex or undifferentiated presentations. Experience of multidisciplinary and integrated team working. Experience of clinical governance, audit or quality improvement.
