Job summary
An exciting opportunity has arisen for a skilled, enthusiastic and motivated Assistant Practitioner to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.
Working within a dynamic multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Administrative Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.
As part of the UCR service, you will support clinicians in responding to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery and safe assistance from the floor.
The successful candidate will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes to adults with physical and mental health needs.
Main duties of the job
The successful candidates will be expected to work holistically alongside the multidisciplinary team. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.
You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.
The ideal candidate would be someone with experience of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients in their own homes.
In return you will be offered a comprehensive induction, access to internal and external training, exposure to a range of clinical expertise within our team, along with regular supervision. If you do not hold the Foundation degree you will be supported to enrol on the Assistant Practitioner foundation degree to aid your future development.

To work alongside Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Support Workers to provide assessment and treatment intervention to people within the Nottingham City Community, who are at risk of hospital admission or to facilitate discharge from hospital.
To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values. You will be working in a variety of settings.
You will participate actively in the services training strategy, including in service training and multi-agency training.

Person Specification
Qualifications Essential
Educated to Foundation degree or equivalent in a health-related discipline or willingness to complete the Assistance Practitioner foundation degree.
Desirable
NVQ/QCF 3 in health and social care GCSE in maths and English to enable the candidate to enrol on the foundation degree course.
Experience Essential
Experience of multi-disciplinary team working in a Health Care, Social Care, Voluntary Care setting. Experience managing own workload in a team setting Experience of working with individuals and/or groups within a Health Care, Social Care, Voluntary Care setting.
