Advanced Nurse Practitioner or Advanced Clinical Practitioner (Paramedic) Full Time/Part Time Considered
Rainbow Medical Centre have a vacancy for an additional Advanced Nurse Practitioner or Advanced Clinical Practitioner (Paramedic) role, as part of our ongoing commitment to expanding and enhancing the services we provide to our patients. This is an excellent opportunity for an experienced advanced practitioner to join a well-established large training practice with a growing list size of approximately 14,850 patients.
Our clinical team include 13 Doctors, 1 ANP, 9 other members of our nursing team (including a Nurse Manager, specialist nurses and HCAs) all supported by a comprehensive admin/reception team.
We are seeking an experienced and motivated clinician to join our friendly and supportive team, combining home visiting with clinical work within the practice.The successful candidate would ideally be able to work on Mondays but if Mondays are problematic than definately Tuesdays
Main duties of the job
The successful candidate will have primary responsibility for triaging our home-visits, offering face to face, same day and urgent home visits for housebound/care home residents, frail elderly or palliative patients, alongside assessing and managing acute presentations within the practice environment.
The role offers an excellent opportunity to work autonomously, delivering comprehensive, patient-centred care to those with complex health needs including proactive frailty management and gold standard framework reviews. The successful candidate will be confident in making independent clinical decisions while recognising when escalation or multidisciplinary input is required.
We are looking for an Independent Prescriber, ideally with experience in general practice, frailty, community or urgent care. The role would suit someone who enjoys variety & is keen to work across both the practice environment and patients' homes.

Salary will be commensurate with experience but is expected to be A4C 8a equivalent.

Job summary

The successful candidate will work both autonomously and collaboratively within the multidisciplinary team, using advanced clinical assessment and decision-making skills to deliver safe, high quality, patient centred care.
Main duties of the job
Key responsibilities include:
Completing home visits for acute, chronic, frail housebound patients
Undertaking clinical sessions within the surgery as and when needed
Managing patients with complex health needs using advanced clinical assessment and diagnostic skills
Supporting palliative and end-of-life care planning, including Gold Standards Framework (GSF) reviews and ReSPECT documentation.
Prescribing and reviewing palliative and end-of-life medications in line with national and local guidance.
Undertaking weekly ward rounds within care homes
Working proactively with patients, relatives, carers and community teams to ensure high-quality care.
Providing holistic assessment, diagnosis, treatment and management plans
Prescribing medication appropriately and safely within scope of practice
Preventing avoidable hospital admissions through proactive management and early intervention.
Maintaining accurate and contemporaneous clinical records using EMIS
Participating in multidisciplinary meetings, safeguarding processes, significant event reviews and clinical audits.
Supporting quality improvement initiative and QOF targets where appropriate.

Undertake regular home visits for acute, chronic, frail, palliative, and housebound patients Carry out weekly ward rounds within nursing and residential care homes Assess, diagnose, treat, and manage patients with complex and undifferentiated conditions Undertake proactive frailty assessments and care planning Support palliative and end-of-life care management, including Gold Standards Framework (GSF) reviews Complete and review ReSPECT forms and advance care planning documentation Prescribe and review medications safely and effectively, including anticipatory and palliative care medications Work collaboratively with GPs, PNs, district nurses, pharmacists, hospice teams, community matrons, social care, and secondary care services. Experience Essential
Significant experience working autonomously within primary care, urgent care, or community settings. Experience managing patients with acute and Chronic conditions. Experience in frailty management and caring for elderly patients. Experience prescribing independently within scope of practice. Experience working within a multidisciplinary team environment. Strong clinical assessment and diagnostic skills.
Desirable
Experience working in general practice. Experience using EMIS and Ardens. Experience of advance care planning and end of life care. Experience supporting palliative care patients and prescribing end of life medications. Experience leading quality improvement initiatives or clinical audits. Experience supporting care home residents. Experience managing palliative care caseloads. Experience participating in Gold Standards Framework meetings.
Qualifications Essential
Advanced Nurse Practitioner or HCPC Registered Paramedic. Current professional registration with NMC or HCPC. Independent Prescribing Qualification. Masters Degree in Advanced Clinical Practice or e-portfolio supported ACP route. Evidence of continued professional development. Full UK driving licence and access to a vehicle for work purposes.
Desirable
Teaching, mentoring or supervision qualification. Qualification or experience in frailty or palliative care. Gold Standards Framework (GSF) or end of life care training.
Working Days Essential
The ideal candidate will be able to work Mondays for the practice but if Mondays are problematic than Tuesday would be the minimum criteria .
