We have an exciting opportunity for a Social Prescriber to join our Primary Care team at HMP Gartree, a category B Prison located in Market Harborough in Leiecestershire.

HMP Stocken is a busy male prison with a capacity for 708 inmates. We are recruiting for a Social Prescriber to support the wider Substance Misuse and wider Healthcare Team with 1-1's with patients, supporting the patients with alternative therapies such as art or wellbeing activities. We are welcoming applications from newly qualified candidates.

Hours

0900 - 1700 Monday - Friday (Can be flexible with start and finish times) Salary

This position is full time, youll receive an annual salary up to £35,000 per annum

Main duties of the job As a Social Prescriber, your responsibilities will vary. You will

Be building professional relationships with the people you are helping by listening carefully to what is important to them and what motivates them. You will be responsible for connecting people to activities and services within the prison, and required to support and motivate patients to access opportunities to help improve their health and wellbeing. This role will see you liaising with key healthcare staff, building and maintaining relationships with a range of prison departments, charities and third sector providers to promote social prescribing and ensure social prescription opportunities are available within your sites.
