We are looking for an experienced Practice Nurse to join our friendly and dedicated Nurse team in a part-time role. The Team is led by an empathetic and skilled Lead Nurse alongside two other Practice Nurses, two Healthcare Assistants and a Phlebotomist.
We are looking for someone with General Practice Nursing experience, ideally across all areas of Long Term Conditions but secure knowledge and support for diabetic patients is essential. If you are still working towards Diabetes as part of your ongoing development, we'd be happy to support your final stages of sign-off to competency.
We are looking for 16-20 hours per week across Mondays and Tuesdays.

Job description Job responsibilities
The key responsibilities are as follows -
Experience of Practice Nurse skills including: long term condition management, wound care, ECG, venepuncture, immunisations, new patient medicals and womans health (cervical cytology, contraception etc.)
Request pathology tests and process the results, advising patients accordingly
Understanding of the importance of evidence-based practice
Ability to promote best practice regarding nursing matters
Ability to work within own scope of practice and understand when to refer to GPs
Good clinical system IT knowledge and an ability to record accurate clinical notes
Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
Understanding of safeguarding adults and children
Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
Experience of working with patients in General Practice is essential. Knowledge of SystmOne (clinical system) would be an advantage. A passion for customer service and a focus on the patient at the heart of everything we do is key.
