The Adur Community Primary Care Network Team is looking to recruit a Registered Nurse to join our friendly and supportive team based in Lancing, providing care to housebound patients across the Adur locality.
Whether you are newly qualified or an experienced nurse seeking a new challenge, Community Nursing offers a rewarding and varied role, including wound care, long-term condition management, palliative care, catheter care, safeguarding, and care home support.

Main duties of the job
As a Band 5 Community Nurse within the Adur PCN Team, you will provide high-quality nursing care to housebound patients in their own homes and care home settings. Duties include:
Assessment, planning, delivery and evaluation of patient care. Wound assessment and management. Catheter and continence care. Administration of medication, including insulin where competent. Supporting patients with long-term conditions and complex health needs. Delivering palliative and end-of-life care. Safeguarding adults and raising concerns where appropriate. Working collaboratively with GPs, therapists, care homes and the wider multidisciplinary team. Supporting hospital discharge and preventing unnecessary hospital admissions. Maintaining accurate clinical records and contributing to care planning. You will receive support and development opportunities to build your community nursing skills and progress your career within a friendly and experienced team.

Job description Job responsibilities
You will see on the Person Specification some of the competencies and expectations you will either have or need to develop if you were successful. The main aspects of the role are Diabetic Management, Wound Management including Leg Ulcers diagnosis and treatment, Palliative care working very closely with the Macmillan and GPs, Bladder and Bowel care including urethral and supra pubic catheters and other continence needs.
As well as the traditional type of patients as stated above, we work in partnership with the local health and social providers covering a specific group of people under Frailty.
Our goal is to deliver high quality care in the community, working with patients towards rehabilitation, self-management and independence throughout the continuum of care. To participate in a team of colleagues supporting evidence based care, facilitating staff learning and development.
