We're looking for an enthusiastic and motivated band 6 practitioner to join our developing Community Rehabilitation Team working across the Bradford and Craven district.
At BDCFT, we're committed to reducing health inequalities and improving outcomes for people living with Severe Mental Illness (SMI) through high-quality, recovery-focused rehabilitation services.
In this role you will be pro-active in reviewing and engaging those with complex psychosis and SMI in their rehabilitation journey .
You will work closely and collaboratively with the Community Mental Health Teams and partner organisations across the district, you'll play a key role in building strong relationships, coordinating care, and ensuring people receive the right support at the right time.

Main duties of the job
To manage a clinical caseload as required to meet the rehabilitation needs of service users.
To support patient flow and development of rehabilitation pathways as defined by the leadership team
To implement evidence-based specialist rehabilitation knowledge and skills
To be a proactive team member in respect of communication, team administration, quality of service and service development.

Experience Essential
Working as an Mental Health Practitioner assessing the needs of service users with severe and enduring mental health problems. Mental health rehabilitation Clear supervisory experience. Professional practice education experience
