Job summary

Clinical Team Manager - Birch Avenue Nursing Home

The postholder will oversee the performance of the team, ensuring supervision, appraisal and appropriate training and development are in place.

To be responsible for the implementation of quality initiative (benchmarking), evaluation, monitoring and clinical audit.

To oversee the ongoing development of the designated Nursing Home and influence Directorate strategic development in response to changing client, local community and national needs.

To ensure care delivery is of the highest standards and is based on evidence/research/best practice and Policies and Procedures.

To provide clinical leadership for the delivery of high-quality dementia and older adult care, ensuring compliance with relevant regulatory standards and best practice frameworks. To support continuous quality improvement, service assurance activities and the maintenance of standards in preparation for internal and external inspection processes.

To manage and make best use of all resources.

To provide effective professional leadership to multidisciplinary team.

In support of providing a co-ordinated and integrated approach to ensure effective working relationships exist between all associated services and agencies.

To provide nursing clinical input into the service, leading in providing health promotional activities, education and training with patients, carers and staff.

Ensure that interventions meet local and national standards and are based on an individualised approach to care, aimed at maximizing an individuals potential and are based on effective engagement principles. Ensure that clear effective communication occurs, with the service user or person identified as the main carer or appropriate contact for each service user. To ensure the maintenance of a safe environment for service users, staff and visitors. To ensure the effective communication and liaison with other agencies. To ensure management of agreed targets. To ensure effective monitoring systems are in place for performance management. To provide effective leadership to the team ensuring team governance. Ensure the assessment and identification of risk and the necessary risk reduction/management measures. Provide effective planning and evaluation of cases Ensure that comprehensive assessment/treatment/interventions are carried out for all assigned service users and carers. Ensure that each service user has a comprehensive, clear and up to date management/treatment plan with specified objectives/treatments designed to meet the service users needs. Ensure the appropriate involvement of relatives/main carers/friends in the formulation and implementation of care plans, while respecting the wishes and autonomy of the service user. Ensure carers are offered an appropriate level of support. Provide expertise in the field of knowledge appropriate to the service and be able to communicate highly sensitive information to service users and carers offering empathy and reassurance. Provide clinical leadership in the care and treatment of older adults with complex mental health needs, frailty and dementia.

Support the development of evidence-based approaches to managing behaviours that challenge, promoting least restrictive practice and person-centred care.

Ensure staff are equipped with the knowledge and skills required to meet the needs of people living with dementia and other complex presentations.

Ensure risk issues for each service user are addressed through regular evaluation Contribute personal and professional skills, to ensure the development and the delivery of the therapeutic group work/interventions Education/Training/Development

To lead in the production of an active learning environment in order to maintain and develop high standards of professional and clinical practice. Promoting opportunities for self, colleagues and junior staff to access available education/training. To plan, prepare and facilitate learning presentations to colleagues, junior staff and other care professionals in health & social care settings. To encourage team members to share their experiences To encourage team members to share identified skills with colleagues, learners and junior staff, identify and lead formal and informal teaching as appropriate and ensure teaching takes place. To take responsibility for identifying personal and team development needs. To lead and participate in appraisal and clinical supervision with junior staff and ensure that this is disseminated and monitored. To ensure the collection and performance review of evidence/competency. To participate in appraisals mechanisms and supervision and receive regular clinical supervision. To participate in the ongoing development of the Directorate. Audit/Evaluation

To take the lead on the development and implementation of clinical audit, unit standards, operational policies and clinical guidelines for the team. To lead and participate in the identification and application of sound research-based evidence to practice. To oversee quality monitoring processes and use information from audits, incidents, safeguarding activity, patient feedback and quality metrics to drive service improvement.

To support the service in demonstrating compliance with applicable regulatory and professional standards.

To assist the Directorate and Trust in the identification of corporate planned developments. To lead and participate in the ongoing review and evaluation of the service. Safeguarding and Quality

To promote a culture of safety, openness and continuous improvement. To ensure safeguarding responsibilities are effectively discharged and that concerns are recognised, escalated and managed in accordance with Trust policy. To contribute to the monitoring of clinical quality indicators, medicines management, infection prevention and control, and patient safety arrangements. Duties & responsibilities

To work flexibly to Service requirements within contracted hours. To work to Professional Codes of Conduct and Trust/Care Directorate Policies and Procedures and ensure all junior staff comply with these standards. To ensure the maintenance of a safe environment in accordance with Health & Safety legislation. To ensure the appropriate delegation of key tasks to junior staff. Be personally responsible for maintaining professional registration with the NMC. Failure to produce evidence of periodic registration on time may lead to regrading/dismissal To ensure that a record is kept of professional nursing registration details of all registered nurses in the clinical area. Training and Qualifications Essential

Registered Nurse - Mental Health

Desirable

Relevant post graduate qualification Management degree or equivalent Diploma in Leadership and Management for Health & Social Care Level 5 or equivalent ( or willing to work towards)

Experience Essential

Experience and evidence of working with older people with mental health problems Demonstrable experience of ability to undertake the role Experience of Service Development & Team Governance Experience in working in partnership with others. Experience in working within MDT/multi professional Experience of working directly with people with Organic illnesses and their carers Knowledge & understanding of mental health issues specifically dementia

Desirable
