About this role

Job summary

We have an exciting opportunity to join our administrative team . The job involves working as part of a team to provide administrative support to help coordinate the care for house bound patients and patients resident in care homes.

You will also contribute towards the creation and maintenance of the care plans. The successful applicant will be computer literate and be able to demonstrate accuracy and attention to detail in their work. The successful candidate must be able to use their initiative and organise their own workload. Multi tasking will be a key part of this role.

Working Hours are

Monday 08:30-15:30

Tuesday 08:30-15:30

Wednesday 08:30-15:30

Thursday 08:30-15:30

Friday 08:30-15:30

The hours can also be condensed in to a 4 day working week if preferred

28.75 hours per week M-F

Main duties of the job

1.Working as part of the Care Coordinator team to provide a single point of contact for all the Kettering Care homes for all non-urgent requests: dedicated phone line, nhs.net email address, Microsoft Teams.

2.Proactively contact all the care homes at least weekly to discuss patient related concerns.

3.Navigate the non-urgent requests and the patient related concerns to the most appropriate service or clinician.

4.Contribute towards the creation and maintenance of the advanced care plans.

5.Ensure all information about the patient is updated using appropriate ARDENs templates and associated read codes.

6.Support alignment of care home to practices, including registration, updating care plans and liaising with clinical pharmacist to ensure medications are updated.

7.Maintain and update records of all patients in care homes who have been admitted to secondary care

8.Coordinate MDTs with the practice GP and community teams.

About us

Weavers Medical is a large GP practice looking after 19,300 patients. We have a team of 18 GPs and a nursing/HC team of 14.

The care coordinator team will help to coordinate the care of housebound and frail patients across 4 practices within the Red Kite Primary Care Network (PCN).

Working from home may sometimes be required.

Responsible to: Practice Manager

Hours: 25-30hpw

Main Purpose:

To provide coordinated care for all patients resident in care homes.

To maintain patient electronic records for clerical reasons, the Quality and Outcomes Framework and for medical care. The records must be accurate and contemporaneous.

Key Tasks

Working as part of the Care Coordinator team to provide a single point of contact for all the Kettering Care homes for all non-urgent requests: dedicated phone line, nhs.net email address, Microsoft Teams.

Proactively contact all the care homes at least weekly to discuss patient related concerns.

Navigate the non-urgent requests and the patient related concerns to the most appropriate service or clinician

Contribute towards the creation and maintenance of the advanced care plans (including when appropriate frailty score, GSF level, date of last SMR, etc).

Ensure all information about the patient is updated using appropriate ARDENs templates and associated read codes.

Support alignment of care home to practices, including registration, updating care plans and liaising with clinical pharmacist to ensure medications are updated.

Maintain and update records of all patients in care homes who have been admitted to secondary care (date admission discharge, reason for admission, post discharge contact.

Coordinate MDTs with the practice GP and community teams.

Manage and prioritise workload on a daily basis and dealing with the competing demands of the care coordinator role.

Other Admin tasks

Attend regular team meetings.

Take part in any training as required from time to time.

Work to the schedule as set by the line manager and report any backlogs of work.

Any other duties considered relevant to the post.
