Job SummaryAbout the Position: This position is located in the Directorate of Mental Health at Naval Health Clinic Annapolis, Annapolis, Maryland. Works as part of a multidisciplinary behavioral health team supporting complex and high-risk patients within the Military Health System. This is a Direct Hire Solicitation for Certain Personnel of the DoD Workforce.

Basic Requirement for Mental Health Case Manager: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Transcript must be provided). In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes performing mental health assessments; coordinating treatment plans; and guiding patients through healthcare systems and mental health resources. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
Major DutiesPerform difficult and complex assessments; conduct interviews; evaluate and analyze health care plan and resources.Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans.Collect and analyze data to identify trends and patterns and makes recommendations for program enhancements.Assess, develop and recommend local operating procedures to meet the unique needs of the clinic, MTF and population served.Communicate with treating facilities, both in direct and purchased care to coordinate patient care and transitions.Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
